玻璃纤维桩与铸造金属桩在口腔残根修复中的效果比较

摘  要:目的比较玻璃纤维桩与铸造金属桩修复残根的效果。方法回顾性分析2015年9月至2016年1月济宁市妇幼保健计划生育服务中心口腔科行残根桩冠修复的173例(211颗牙)患者的临床资料,按照患者意愿分为玻璃纤维桩组(n=102,124颗牙)与铸造金属桩组(n=71,87颗牙)。两组均行烤瓷全冠修复,玻璃纤维桩组应用玻璃纤维桩进行残根修复,铸造金属桩组应用铸造金属桩进行残根修复,比较两组修复前后患牙咬合功能、牙周健康水平、疼痛程度及不良反应发生情况。结果修复前,两组咬合力水平、龈沟出血指数(SBI)评分、牙龈指数(GI)评分、疼痛评分比较差异无统计学意义;修复3年后,两组咬合力水平均高于修复前,SBI、GI、疼痛评分均低于修复前,且玻璃纤维桩组咬合力水平高于铸造金属桩组,SBI、GI、疼痛评分均低于铸造金属桩组,差异有统计学意义(P<0.05)。玻璃纤维桩组不良反应发生率为4.03%,低于铸造金属桩组的14.94%,差异有统计学意义(P<0.05)。结论玻璃纤维桩用于残根修复的临床性能优于铸造金属柱,临床修复效果更理想,不良反应发生率低,具有更高的临床应用潜能。Objective To compare the effects of glass fiber post and cast metal post in repairing residual roots.Methods The clinical data of 173 patients(211 teeth)who underwent residual root post crown restoration in the department of Stomatology of Jining Maternal and Child Health Family Planning Service Center from September 2015 to January 2016 were retrospectively analyzed,they were divided into the glass fiber post group(n=102,124 teeth)and cast metal post group(n=71,87 teeth)according to the wishes of the patients.Both groups were restored with porcelain crown,the glass fiber post group was restored with glass fiber post,and the cast metal post group was restored with cast metal post.The occlusal function,periodontal health level,pain degree and the occurrence of adverse reactions were compared between the two groups.Results Before restoration,there were no significant differences in the bite force level,the sulcus bleeding index(SBI),gingival index(GI)and pain scores between the two groups;after 3 years of restoration,the bite force level of the two groups was higher than that before restoration,and the SBI,GI,and pain scores were lower than those before restoration,and the bite force level in the glass fiber post group was higher than that in the cast metal post group,the scores of SBI,GI and pain were lower than those in the cast metal post group,and the differences were statistically significant(P<0.05).The incidence of adverse reaction was 4.03%,which was lower than 14.94%in the cast metal post group,and the difference was statistically significant(P<0.05).Conclusion The clinical performance of glass fiber post in residual root restoration is better than that of cast metal post,with better clinical restoration effect,lower incidence of adverse reactions and high clinical application potential.
