摘  要:目的探讨菊粉型益生元对2型糖尿病(T2DM)患者免疫细胞及炎症状态的调理效果。方法选取该院内分泌科住院的T2DM患者59例作为研究对象,根据患者入院顺序结合患者接受口服菊粉型益生元治疗的意愿分为观察组(24例)和对照组(35例)。对照组按照指南进行规范诊断和治疗,观察组由营养师根据患者病情及饮食习惯制订个性化营养治疗方案,并口服菊粉型益生元,分析两组患者糖脂代谢、营养免疫状态及炎症因子的变化。结果两组治疗后空腹血糖、糖化血红蛋白、胰岛素抵抗指数、甘油三酯水平与治疗前比较均明显下降,同时高密度脂蛋白胆固醇水平明显升高,差异均有统计学意义(P<0.05),总胆固醇、低密度脂蛋白胆固醇水平无明显变化,差异均无统计学意义(P>0.05)。对照组常规治疗后的前清蛋白(PALB)水平高于治疗前,差异有统计学意义(t=6.02,P<0.05)。观察组经过菊粉型益生元干预治疗后PALB水平、T淋巴细胞亚群(CD3^(+)T淋巴细胞、CD3^(+)CD4^(+)T淋巴细胞和CD3^(+)CD8^(+)T淋巴细胞)绝对值计数、清蛋白水平均高于治疗前,差异均有统计学意义(P<0.05)。两组治疗后超敏C反应蛋白(hs-CRP)、白细胞介素-6水平明显均低于同组治疗前,且观察组hs-CRP水平明显低于对照组,差异均有统计学意义(P<0.05)。结论菊粉型益生元可以调节T2DM患者的糖脂代谢、免疫细胞及炎症因子水平,值得临床进一步推广使用。Objective To investigate the effects of inulin prebiotics on immune cells and inflammatory state in patients with type 2 diabetes mellitus(T2DM).Methods A total of 59 patients with type 2 diabetes mellitus were divided into observation group(24 cases)and control group(35 cases)according to the order of admission and the willingness to take oral inulin prebiotics.The control group was given standardized diagnosis and treatment according to the guidelines,while the observation group was given individualized nutritional treatment by nutritionist according to the patient's condition and dietary habits,and oral inulin prebiotics.The changes of glucose and lipid metabolism,nutritional immune status and inflammatory factors were compared between the 2 groups.Results The fasting blood glucose,glycosylated hemoglobin,insulin resistance index and triglycerides levels of the 2 groups after treatment were significantly lower than those before treatment,while high density lipoprotein cholesterol level was significantly increased,the differences were statistically significant(P<0.05),total cholesterol and low density lipoprotein cholesterol levels had no significant change,and the differences were not statistically significant(P>0.05).The prealbumin(PALB)level of control group after routine treatment was higher than that before treatment,and the difference was statistically significant(t=6.02,P<0.05).After inulin prebiotics intervention,PALB,T lymphocyte subsets(CD3^(+)T lymphocytes,CD3^(+)CD4^(+)T lymphocytes and CD3^(+)CD8^(+)T lymphocytes)absolute count and albumin were significantly higher than before treatment in observation group,and the differences were statistically significant(P<0.05).The levels of high-sensitivity C-reactive protein(hs-CRP)and interleukin-6 in the 2 groups were significantly lower than those before treatment,and the level of hs-CRP in observation group was significantly lower than that in control group,the differences were statistically significant(P<0.05).Conclusion Inulin prebiotics can regulate gl
