摘  要:目的基于国家儿童肿瘤监测数据,描述我国儿童及青少年脑(脊)膜瘤新发病例的人群分布、原发部位和肿瘤性质等临床流行病学特征。方法采用国家儿童肿瘤监测中心2018-2020年新发肿瘤病例数据库,根据儿童肿瘤国际分类标准第三版调取0~19岁的新发脑(脊)膜瘤患儿病例信息,包括性别、确诊年龄、疾病诊断、原发部位、肿瘤性质。将新发病例划分为4个年龄组:0~4岁(幼儿组)、5~9岁(儿童组)、10~14岁(青春期早期组)和15~19岁(青春期晚期组),分析我国不同性别、年龄组脑(脊)膜瘤新发病例的原发部位、肿瘤性质分布特征。使用χ^(2)检验或Fisher精确概率法评估脑(脊)膜瘤肿瘤性质在不同性别及年龄组新发病例中的分布差异。结果428例脑(脊)膜瘤新发患儿中男孩228例,女孩200例,在不同性别的儿童及青少年中,病例数量的占比均随年龄升高而增加。374例(87.38%)为脑膜瘤,54例(12.62%)为脊膜瘤;额叶(54例,12.62%)、脑室(50例,11.68%)和颞叶(46例,10.75%)为脑膜瘤最常见的原发部位。317例(74.07%)为良性肿瘤,75例(17.52%)为交界恶性肿瘤,36例(8.41%)为恶性肿瘤;在不同年龄组中,肿瘤性质分布差异有统计学意义,P<0.001;0~4岁幼儿组的交界恶性及恶性肿瘤占比分别达到35.00%和22.50%,随着年龄的增加,恶性和交界恶性肿瘤占比呈下降趋势;在不同原发部位的脑(脊)膜瘤中,脑干部位脑膜瘤的恶性比例(18.18%)最高。结论儿童及青少年脑膜瘤好发于额叶、脑室和颞叶等部位。相较于青少年,儿童中恶性及交界恶性脑(脊)膜瘤的占比更高,特别是0~4岁幼儿。Objective Based on data from the national pediatric cancer surveillance,we aimed to describe the epidemiological characteristics of the population distribution.primary site,and tumor nature of newly diagnosed meningiomas among children and adolescents in China.Methods This study is a nationwide cross-sectional study.Using new incidence case database for 2018-2020 from the National Center for Pediatric Cancer Surveillance,according to the third edition of the International Classification of Childhood Cancer,case information of newly diagnosed meningioma patients aged 0-19 years old was extracted,including sex,diagnostic age,disease diagnosis,primary site,and tumor nature.We classified the age of patients into four age groups;0-4 years(preschool group),5-9 years(children group),10-14 years(early puberty group),and 15-19 years(late puberty group),and analyzed the distribution characteristics of the primary site and tumor nature of meningiomas in different sex and age groups of new cases in China.The Chi-square test or Fisher's exact test was used to evaluate the distribution differences of tumor nature in different sex and age groups of new cases with meningiomas.Results Among 428 new cases of meningioma,228 were boys and 200 were girls.The proportion of new cases increased with age both in boys and girls.374 cases(87.38%)were intracranial meningiomas,and 54 new cases(12.62%)were spinal meningiomas.The frontal lobe(54 new cases,12.62%),ventricles(50 new cases,11.68%),and temporal lobe(46 new cases,10.75%)were the top three common primary sites of meningionas.317 new cases(74.07%)were benign tumors,75 new cases(17.52%)were borderline malignant tumors,and 36 new cases(8.41%)were malignant tumors.The distribution of tumor nature had significantly difference among different age groups(P<0.001).Borderline malignant tumors and malignant tumors in the 0-4 years old group of children accounted for 35.00%and 22.50%,respectively.With the increase of age,the proportion of malignant and borderline malignant tumors decreased.Amo
