摘 要:目的:检测食管癌患者血清微管解聚蛋白(Stathmin)、肿瘤特异性生长因子(TSGF)、凝血酶敏感蛋白-1(TSP-1)水平与其放疗效果的关联性。方法:选取2018年1月—2023年3月某院收治的115例食管癌患者为研究对象,根据放疗4周后临床疗效将其分为无效组(26例)和有效组(89例)。检测所有患者放疗前后血清Stathmin、TSGF、TSP-1水平,分析放疗前后血清Stathmin、TSGF、TSP-1水平差值(△值)与放疗效果的相关性,多因素Logistic回归方程分析影响放疗效果的因素,并分析不同△Stathmin、△TSGF、△TSP-1水平对放疗效果的危险度。结果:放疗4周后,有效组血清Stathmin、TSGF[(2.45±0.63)μg/L、(52.35±7.28)U/L]水平低于无效组[(7.08±2.06)μg/L、(98.71±11.62)U/L],血清TSP-1[(196.42±12.28)ng/mL]水平高于无效组[(109.33±11.09)ng/mL],有效组血清△Stathmin[(7.18±1.44)μg/L]、△TSGF[(62.41±5.84)U/L]、△TSP-1[(113.27±10.24)ng/mL]水平均高于无效组[(2.66±0.57)μg/L、(17.13±3.42)U/L、(26.87±5.38)ng/mL)],差异有统计学意义(P<0.05);Spearman相关分析显示,血清△Stathmin、△TSGF、△TSP-1值与放疗效果呈正相关(P<0.05);Logistic回归分析显示,血清△Stathmin、△TSGF、△TSP-1值高水平为影响食管癌患者放疗效果的保护因素(P<0.05);血清△Stathmin、△TSGF、△TSP-1高水平患者放疗无效的危险度是低水平的0.333、0.241、0.194倍(P<0.05)。结论:血清Stathmin、TSGF、TSP-1水平在食管癌患者放疗前后变化显著,且变化差值与放疗效果存在正相关关系,为放疗效果的影响因素,临床可通过检测其水平预测远期放疗效果。Objective:To detect the levels of serum stathmin,tumor supplied group of factors(TSGF),thrombospondin-1(TSP-1)of patients with esophageal carcinoma and explore their relationship with the effect of radiotherapy.Methods:115 patients with esophageal carcinoma treated in a hospital from January 2018 to March 2023 were recruited in the study and,according to the clinical effect after 4 weeks'radiotherapy,were divided into an ineffective group(n=26)and an effective group(n=89).Levels of serum stathmin,TSGF,TSP-1 of all the patients before and after the radiotherapy were detected and the correlation between the difference values(△value)and the effect of the radiotherapy was analyzed.Multivariate logistic regression analysis was used to explore the factors influencing the effect of the radiotherapy,and the risk degree imposed by the different levels of△stathmin,△TSGF,and△TSP-1 on the effect of the radiotherapy was investigated.Results:4 weeks after the radiotherapy,levels of serum stathmin and TSGF in the effective group[(2.45±0.63)μg/L,(52.35±7.28)U/L]were lower than those of the ineffective group[(7.08±2.06)μg/L,(98.71±11.62)U/L],while level of serum TSP-1[(196.42±12.28)ng/mL]was higher than that of the ineffective group[(109.33±11.09)ng/mL].Serum△stathmin[(7.18±1.44)μg/L],△TSGF[(62.41±5.84)U/L],and△TSP-1[(113.27±10.24)ng/mL]of the effective group were all higher than those of the ineffective group[(2.66±0.57)μg/L,(17.13±3.42)U/L,(26.87±5.38)ng/mL](P<0.05).Spearman correlation analysis showed that serum△stathmin,△TSGF and△TSP-1 were positively correlated with the effect of the radiotherapy(P<0.05).Logistic regression indicated that high levels of△stathmin,△TSGF and△TSP-1were protective factors influencing the effect of the radiotherapy for the patients with esophageal carcinoma(P<0.05);the risk degree in the ineffective radiotherapy of the patients with high△stathmin,△TSGF and△TSP-1 were 0.333,0.241,0.194 times of the patients with low levels of△stathmin,△TSGF and△
