摘 要:目的:探索不同类别SP在全科住院医师培养中的应用价值;方法:选择安徽医科大学2017级全科医学专业120名学生,随机分为4组,分别是3个试验组(PSP组、TSP组、SSP组)以及1个对照组(传统课堂教学),每组各30人,根据教学大纲对各组学生进行10学时的指导(SSP组包括教师做少量指导及勘误),在指导前后各进行一次接诊,并接受打分,评分标准包括问诊、查体、监考教师提问、人文关怀4个部分。同时对3个试验组学生进行问卷调查,以反映过程中其实际收获;结果:试验后,PSP组、TSP组、SSP组总成绩分别为(83.82±6.33)分、(90.97±4.92)分、(87.99±5.66)分,均明显高于对照组的(74.11±10.05)分,且均高于实验前数据,其中TSP组的查体、人文部分得分最高,为(23.14±1.44)分、(21.98±3.66)分,与其他组相比差异有统计学差异(P<0.05)。结论:SP教学方式较传统教学方式能够明显增加全科住院医师的临床胜任能力,TSP组在目前阶段所有SP教学方法中能够取得更好的效果。Objective:To explore the application value of different categories of standardized patients(SP)in general medical resident training.Methods:A total of 120 students from the 2017 class of the general medicine major at Anhui Medical University were randomly divided into four groups:three experimental groups(the PSP group,TSP group,SSP group)and a control group(traditional classroom teaching),with 30 students in each group.Each group received 10 hours of guidance according to the teaching outline.The SSP group included limited teacher guidance and correction.Each group conducted one patient consultation before and after the guidance sessions and received ratings based on four criteria:inquiry,physical examination,teacher questioning,and humanistic care.Additionally,a questionnaire survey was conducted among students in the three experimental groups to reflect their actual learning experience.Results:After the experiment,the total scores for the PSP group,TSP group,and SSP group were(83.82±6.33),(90.97±4.92)and(87.99±5.66)points,respectively,all significantly higher than the control group's(74.11±10.05)points.Furthermore,the scores in all three experimental groups improved compared to their pre-experiment scores.The TSP group had the highest scores in the physical examination and humanistic care categories,with(23.14±1.44)and(21.98±3.66)points,respectively,showing statistical significance(P<0.05).Conclusion:SP teaching methods are more effective in enhancing the clinical competence of general medical residents compared to traditional teaching methods.Among the different SP teaching methods,the TSP group achieved the best results at this stage.
