摘  要:目的 分析剥夺睡眠脑电图(sdEEG)联合血清谷氨酰胺、胰岛素样生长因子-1(IGF-1)在儿童癫痫诊断中的应用价值。方法 选取2020年3月—2022年3月收治的86例疑似癫痫患儿作为观察对象,均进行sdEEG检查。分析sdEEG对癫痫检出情况,比较sdEEG对不同年龄段患儿癫痫样放电的检出率及出现癫痫样放电时间,同时比较癫痫与非癫痫患儿血清谷氨酰胺、IGF-1水平。采用受试者工作特征曲线评估sdEEG、血清谷氨酰胺、IGF-1对儿童癫痫的诊断价值。结果 86例疑似癫痫患儿确诊癫痫74例(86.05%)。sdEEG诊断癫痫66例,非癫痫20例,检出率为76.74%。sdEEG对1~3岁患儿癫痫样放电的检出率高于4~7岁和>7岁患儿(P<0.05);患儿睡眠期癫痫样放电占比高于清醒期和思睡期(P<0.01)。癫痫患儿血清谷氨酰胺、IGF-1水平低于非癫痫患儿(P<0.01)。sdEEG、血清谷氨酰胺和IGF-1诊断儿童癫痫的曲线下面积(AUC)分别为0.849、0.740、0.804,三者联合检测AUC为0.735。结论 sdEEG适用于各年龄阶段癫痫患儿的检查,sdEEG、血清谷氨酰胺和IGF-1对儿童癫痫均具有较高的诊断价值。Objective To analyze the application value of sleep-deprivation electroencephalogram(sdEEG) combined with serum glutamine and insulin-like growth factor-1(IGF-1) in children with epilepsy.Methods A total of 86 children with suspected epilepsy admitted to hospital from March 2020 to March 2022 were selected as observation subjects,all of whom underwent sdEEG examination.The detection of epilepsy by sdEEG was analyzed,the detection rate of epileptic discharge and the time of epileptic discharge by sdEEG in children of different ages were compared,and the serum glutamine and IGF-1 levels were compared between epileptic and non-epileptic children.The diagnostic value of sdEEG combined with serum glutamine and IGF-1 in children with epilepsy was evaluated by receiver operating characteristic(ROC) curve.Results Of the 86 children with suspected epilepsy,74(86.05%) were diagnosed with epilepsy.Sixty-six patients with epilepsy and 20 patients without epilepsy were diagnosed by sdEEG,and the detection rate was 76.74%.The detection rate of epileptic discharge in children aged 1 to 3 years was the highest by sdEEG,which was significantly higher than that in children aged 4 to 7 years and >7 years(P<0.05).The proportion of epileptic discharge in sleep period was significantly higher than that in awake period and sleepiness period by sdEEG(P<0.01).Serum levels of glutamine and IGF-1 in epileptic children were significantly lower than those in non-epileptic children(P<0.01).The area under the ROC curve(AUC) of sdEEG,serum glutamine and IGF-1 in the diagnosis of childhood epilepsy were 0.849,0.740 and 0.804,respectively,and the AUC of the combined detection of the three was 0.735.Conclusion sdEEG is appropriate for the detection of epilepsy in children of all ages.sdEEG,serum glutamine and IGF-1 are of high value in the diagnosis of epilepsy in children.
