浙江省医疗保险保障水平与医疗卫生服务水平的匹配度研究

摘　　要：目的:测算与评估浙江省医疗保险保障水平与医疗卫生服务水平的匹配度,为医疗卫生服务水平与医疗保险保障水平的协调发展提供参考。方法:以浙江省11个地级市为研究对象,利用灰色关联度求权重法和基于熵权法的Topsis,测算评估医疗保险保障水平与医疗卫生服务水平之间的匹配度。结果:从医疗保险保障来看,浙江省医疗保险保障广度、深度、效率数值分别处于0.0485-0.1478区间、0.0640-0.1541区间、0.0531-0.1414区间,区间跨度较大;从医疗卫生服务来看,浙江省医疗卫生服务广度和深度指标区间跨度分别达到0.2526、0.2863。杭州综合得分(0.2731),显著高于其他城市。从总体来看,浙江省各地级市医疗保险保障水平与医疗卫生服务水平的匹配度得分有所差异,区间跨度达0.1542。结论:浙江省医疗保险保障分布不均衡;医疗卫生服务空间差异明显;医疗保险保障水平和医疗卫生服务水平存在不匹配,具体表现为医疗卫生服务滞后影响医疗保险保障效用提升、医疗保险保障不足影响医疗卫生服务供给两方面。建议通过政府支持、医疗卫生服务资源下沉、机构数据互通实现优化改善。Objective:To measure and evaluate the matching degree between the level of medical insurance protection and the level of medical and health services in Zhejiang province,so as to provide reference for the coordinated development of the level of medical and health services and the level of medical insurance.Methods:Using 11 prefecture-level cities in Zhejiang province as research objects,the matching degree between the level of medical insurance protection and the level of medical and health services was measured and evaluated using the grey correlation degree weighting method and the Topsis based on the entropy weighting method.Results:From the point of view of medical insurance coverage,medical insurance breadth,depth,and efficiency values in Zhejiang province were in the 0.0485-0.1478 interval,0.0640-0.1541 interval and 0.0531-0.1414 interval,and interval span was large.From the point of view of medical and health services,medical and health services breadth and depth of the index of the interval span in Zhejiang province reached 0.2526 and 0.2863 respectively.Hangzhou's comprehensive score(0.2731)was significantly higher than other cities.Overall,the scores of insurance coverage and health service matching varied among prefecture-level cities in Zhejiang province,with an interval span of 0.1542.Conclusion:Zhejiang province medical insurance protection distribution is not balanced,medical and health service space difference is obvious,and medical insurance protection level and medical and health service level mismatch exists,specifically manifested in medical and health services lagging behind affecting medical insurance protection utility enhancement,medical insurance protection insufficient to affect the supply of medical and health services in two aspects.It is suggested that optimisation and improvement should be achieved through government support,the sinking of medical and health service resources,and the interoperability of institutional data.
