新型揿针结合普通针刺治疗肌张力低下型脑瘫(脾胃虚弱型)的临床疗效观察

摘　　要：目的:探讨新型揿针结合普通针刺治疗肌张力低下型脑瘫(脾胃虚弱型)的临床效果。方法:选取2022年1~12月康复科收治的肌张力低下型脑瘫(脾胃虚弱型)患儿50例作为研究对象,采用随机数字表法分为甲组与乙组,各25例。乙组采用普通针刺治疗,甲组采用新型揿针结合普通针刺治疗。治疗前后采用粗大运动功能测试量表-66(GMFM-66)、徒手肌力评定量表(MMT)、改良Asworth量表、Peabody运动发育量表评估两组运动功能、肌张力等情况,并比较两组下肢活动度、中医证候积分及临床疗效。结果:治疗后,甲组GMFM-66、MMT、Peabody、改良Asworth评分及足背屈曲、内收肌角、腘窝角活动度均优于乙组,差异有统计学意义(P<0.05);甲组中医主症与次症评分均低于乙组(P<0.05);甲组临床有效率为88%,高于乙组的56%,差异有统计学意义(P<0.05)。结论:揿针疗法结合普通针刺治疗对改善肌张力低下型脑瘫患儿的运动功能、肌张力、关节活动度均具有较好疗效。Objective:To investigate the clinical effects of new type of press-acupuncture combined with ordinary acupuncture in the treatment of hypotonic cerebral palsy(spleen and stomach weakness type).Methods:Fifty children with hypotonic cerebral palsy(spleen and stomach weakness type)treated in the rehabilitation department from January 2022 to December 2022 were selected as the research subjects,and were divided into group A and group B with 25 cases each by random number table method.Group B was treated with ordinary acupuncture,group A was treated with new type of press-acupuncture combined with ordinary acupuncture.Before and after treatment,the motor function and muscle tone of the two groups were evaluated by Gross Motor function Test Scale-66(GMFM-66),Manual Muscle Strength Rating Scale(MMT),modified Asworth scale and Peabody Motor Development Scale.The lower extremity mobility,TCM symptom score and clinical efficacy were compared between the two groups.Results:After treatment,GMFM-66,MMT,Peabody,modified Asworth scores,dorsalis flexion,adductor angle and popliteal angle of group A were higher than those of group B,with statistically significant differences(P<0.05).The scores of main and secondary symptoms in group A were lower than those in group B(P<0.05).The clinical effective rate of group A was 88%,higher than that of group B(56%),and the difference was statistically significant(P<0.05).Conclusion:New type of press-acupuncture combined with ordinary acupuncture therapy has good effects on improving motor function,muscle tone and joint motion of children with hypotonic cerebral palsy.
