摘 要:目的:探讨微种植体支抗对安氏Ⅰ类及Ⅱ类上颌前突青少年患者的矫治效果。方法:回顾性分析2019年6月-2022年1月笔者医院就诊的60例安氏Ⅰ类及Ⅱ类上颌前突青少年患者的临床资料。按照治疗方式分组:对照组(30例)给予传统支抗治疗,研究组(30例)给予微种植体支抗治疗。比较两组治疗前后头影测量项目及咀嚼功能变化,统计矫治时间及不良反应发生情况。结果:治疗后6个月,研究组ANB角、U1-NA角、U1-NA、U1-X、U1-Y、A-Y、UL-Y、L1-Y低于对照组,鼻唇角、U1-L1角高于对照组(P<0.05);研究组上切牙内收量(6.82±2.03)mm,高于对照组的(4.65±1.12)mm(P<0.05)。治疗后1年,研究组牙齿咬合力、咀嚼效率高于对照组(P<0.05);治疗期间,研究组牙齿排齐整平时间、拔牙间隙关闭时间及精细调整时间短于对照组(P<0.05);研究组不良反应发生率为6.67%,低于对照组的26.67%(P<0.05)。结论:微种植体支抗对安氏Ⅰ类及Ⅱ类上颌前突青少年患者有较好矫治效果,能促进前牙内收,改善咀嚼功能,且矫治时间短,不良反应少。Objective To investigate the effect of micro-implant anchorage on the treatment of ClassⅠand ClassⅡmaxillary protrusion in teenagers patients.Methods The clinical data of 60 young patients with ClassⅠand ClassⅡmaxillary protrusion in author’s hospital from June 2019 to January 2022 were retrospectively analyzed.Grouped by treatment type:The control group(30 cases)received traditional anchorage therapy,and the study group(30 cases)received micro-implant anchorage therapy.The changes of cephalic imaging and masticatory function before and after treatment were compared between the two groups.The time of treatment and the occurrence of adverse reactions were statistically analyzed.Results Six months after surgery,the ANB Angle,U1-NA Angle,U1-NA,U1-X,U1-Y,A-Y,UL-Y and L1-Y of the study group were lower than those of the control group,while the nasolabial Angle and U1-L1 Angle of the study group were higher than those of the control group(P<0.05).The adduction volume of upper incisor teeth in the study group was(6.82±2.03)mm,which was higher than that in the control group(4.65±1.12)mm(P<0.05).One year after surgery,the biting force and masticatory efficiency of the research group were higher than those of the control group(P<0.05).The teeth leveling time,extraction space closing time and fine adjustment time of the study group were shorter than those of the control group during treatment(P<0.05).The incidence of adverse reactions in the study group was 6.67%,which was lower than that in the control group 26.67%(P<0.05).Conclusion Micro-implant anchorage has a better therapeutic effect on the adolescent patients with ClassⅠand ClassⅡmaxillary protrusion,which can promote anterior adduction,improve mastication function,and the treatment time is short with less adverse reactions.
