摘　　要：目的探讨基于知识转化模式(KTA)构建的口腔癌围术期患者免疫营养支持管理方案,以期为临床相关治疗提供参考。方法采用KTA理论框架,检索常用数据库及指南网中有关口腔癌围术期免疫营养的临床实践指南、证据总结、专家共识、系统评价及综述,按步骤进行文献筛选、评价、提取证据、评估阻碍因素,形成口腔癌围术期患者免疫营养支持管理方案。结果共纳入16篇文献,包括指南5篇、专家共识2篇、循证实践5篇、证据总结1篇、综述3篇,汇总了37条证据,分为营养风险筛查和营养评估、术前营养管理、术后营养管理、围术期免疫营养管理、出院指导五部分,构建口腔癌免疫营养干预方案及实施流程。结论基于KTA构建的免疫营养支持方案对口腔癌患者的术后康复具有一定指导作用,可进行推广应用。Objective This paper aims to explore the immunonutritional support management program for perioperative patients with oral cancer based on the Knowledge to Action Framework(KTA)to provide evidence for clinical treatment.Methods The clinical practice guidelines,evidence summary,expert consensus,systematic review,and review of perioperative immunonutrition for oral cancer in the commonly used databases and guideline networks were searched based on the KTA.The literature screening,evaluation,evidence extraction,and obstructive factors of evaluation were performed following the steps to revise the perioperative immunonutritional support management program for oral cancer.Results A total of 16 articles were included,including 5 guidelines,2 expert consensus,5 evidence-based practice,1 evidence summary,and 3 reviews.Overall,37 items of evidence were summarized and divided into five parts:nutritional risk screening and nutritional assessment,preoperative nutritional management,postoperative nutritional management,perioperative immunonutritional management,and discharge instructions to establish an immunonutritional intervention program and implementation process for this disease.Conclusion The KTA-based immunonutritional support program plays a guiding role in the postoperative rehabilitation of patients with oral cancer,justifying a wider application.
