摘　　要：报告1例色素性乳房Paget’s病。患者女,63岁,左侧乳头变黑2个月。皮肤科检查:左侧乳头弥漫变黑,表面相对光滑,基底颜色略呈褐色,色素欠均匀,乳头中央稍凹陷,表面少许鳞屑,乳头外侧乳晕部分变黑。皮损组织病理:表皮棘层增厚,基底层及棘层中上部较多Paget细胞,其间较多黑素颗粒,真皮浅层较多淋巴细胞及噬色素细胞浸润。免疫组化:细胞角蛋白(CK)7(+),CK20(-),MelanA少量(+),上皮细胞膜抗原(EMA)(+),S-100蛋白(-),高迁移组转录因子(SOX-10)(-)。诊断:色素性乳房Paget’s病,治疗:行左侧全乳房切除术。A case of pigmented mammary Paget’s disease was reported.The patient was a 63-year-old female with the darkening of left nipple for 2 months,Histopathology showed that the epidermis was thickened with the Paget cells involving the basal layer as well as the middle and upper spinous layer.Melanin granules were noted.There are increased numbers of lymphocytes and melanophages in the superficial dermis.Immunohistochemical study showed that the tumor cells were positive for CK7,EMA,and are negative for CK20,MelanA(A few+),S-100 and SOX-10.Combined with the clinical,histopathological and immunohistochemical findings,a diagnosis of pigmented mammary Paget’s disease is made.The patient was treated with left total mastectomy.
