摘 要:目的:调查广州市基层医疗卫生机构全科医生工作满意度并分析其影响因素,为均衡提高基层医疗卫生服务水平提供参考。方法:数据源自2021年广州市基层医疗卫生机构服务状况公众评价调查,纳入广州市48所基层医疗卫生机构的1043名全科医生,对数据进行描述性统计分析、单因素分析和多重线性回归分析以识别全科医生工作满意度的影响因素,进一步使用敏感性分析证明结果的稳健性。结果:全科医生工作满意度平均得分为(3.98±0.95),工作满意率为53.6%,工作本身维度、工作回报维度、医院组织维度、医院内工作环境维度、社会职业环境维度满意率依次为60.7%、55.8%、61.4%、59.3%、58.7%。辖区、编制类型、工作年限、夜班次数、家人支持度及工作热爱程度是全科医生工作满意度的影响因素(P<0.05)。结论:广州市全科医生工作满意度相对较高,但仍可通过深化收入分配改革、加强基层医疗卫生机构硬件建设、健全晋升评价机制、增设编制、调整工作时长、提升社会认可度的方式,进一步提升基层医疗卫生机构全科医生的工作满意度。Objective:To investigate the situation of job satisfaction of general practitioners in grassroots medical and health institutions and analyze its influencing factors in Guangzhou,so as to provide reference for improving the quality of grassroots medical and health services in a balanced manner.Methods:The data were derived from the survey of 2021 public evaluation of service conditions of grassroots medical and health institutions in Guangzhou,which included 1043 general practitioners from 48 grassroots medical and health institutions in Guangzhou.The data were analyzed by descriptive statistical analysis,univariate analysis and multiple linear regression analysis in order to identify the influencing factors of job satisfaction of general practitioners.Besides,sensitivity analysis was further used to prove the robustness of the above conclusions.Results:The overall job satisfaction score of general practitioners was(3.98±0.95),and 53.6%of them were satisfied with their current work.Respectively,their job satisfaction rates in the dimensions of work itself,work return,hospital organization,hospital working environment and socio-professional environment were 60.7%,55.8%,61.4%,59.3%and 58.7%.The working district,status of budgeted posts,working years,frequency of night shifts,degree of family support and love for work were the influencing factors of job satisfaction of general practitioners(P<0.05).Conclusion:The satisfaction of general practitioners in Guangzhou is relatively high.To further increase their job satisfaction,it is necessary to further deepen the reform of the income distribution system for general practitioners,strengthen the hardware construction of grassroots medical and health institutions,improve the promotion and evaluation mechanism,increase the number of staff,adjust the working hours and enhance social recognition.
