摘  要:目的 利用Mini-CEX评分表格,让实习医师以“学生考官”和“考生”双重身份进行考核,完成临床能力评估,为提升学生学习自主性的方法提供参考。方法 选取2021年7月1日-2022年5月31日间37名在某医科大学附属医院心内科实习的2017级临床医学专业本科生,37名实习医师按医院轮转要求分为3组,每组入科时间间隔3个月,每组分别有10、12和15人。所有实习医师以“学生考官”和“考生”双重身份参加Mini-CEX考核,考核后立即由教师考官对“学生考官”和考生进行现场反馈,共30分钟。通过发放问卷调查评价教学效果。结果 97.3%的实习医师和全部教师考官认为Mini-CEX对临床实践的评价客观,所有实习医师认为及时反馈有助于提高临床能力,81.1%的实习医师和80%的教师考官认为让学生做“考官”可以提升学习自主性。83.8%的实习医师认为当“考官”和“考生”有区别,85%的教师考官认为让学生做“考官”对临床实习有帮助可以提升学习自主性。在面谈技巧、沟通技巧、人文关怀、组织效能和整体表现项目,各组平均分呈现后入科组优于前入科组的趋势。结论 利用Mini-CEX让学生做“考官”,可提升学习自主性,提升在医疗面谈技巧、人文关怀、临床判断、体格检查、沟通技能、组织效能、整体临床胜任能力方面的表现。Objectives With the application of the Mini CEX scoring table,interns were assessed as both"student examiners"and"candidates"to complete clinical competency assessments and explore whether this method can enhance learning autonomy,and provide reference for improving students'learning autonomy.Methods 37 undergraduate students majoring in clinical medicine who interned in the Department of Cardiology at a certain affiliated hospital of a medical university between July 1st,2021 and May 31st,2022,were selected to participate in the Mini CEX assessment as both"student examiners"and"candidates".After the assessment,the teacher examiner immediately provided on-site feedback to the"student examiners"and candidates for a total of 30 minutes.To evaluate teaching effectiveness through distributing questionnaires.Results 36(97.3%)intern doctors and 20(100%)teacher examiners believed that Mini CEX's evaluation of clinical practice was objective,37(100%)intern doctors and 20 teachers(100%)believed that timely feedback would help improve clinical abilities,and 30(81.1%)intern doctors and 16(80%)teacher examiners believed that having students act as"examiners"can enhance learning autonomy.31 interns(83.8%)believed that there was a difference between being an"examiner"and a"candidate",while 17 teacher examiners(85%)believed that having students become"examiners"was helpful for clinical internships and could enhance learning autonomy.In terms of interview skills,communication skills,humanistic care,organizational effectiveness,and overall performance,the average scores of each group showed a trend of the latter group being better than the former group.The improvement trend in physical examination items was not significant,while there was no improvement trend in clinical judgment items.Conclusions With the application of Mini-CEX to allow students to be"examiners"could enhance learning autonomy,improve performance in medical interview skills,human care,clinical judgment,physical examination,communication skills,organizational eff
