摘  要:目的 分析肺腺癌组织中前列腺素内过氧化物合酶2(PTGS2)表达水平与表皮生长因子受体(EGFR)基因突变的相关性。方法 选取57例肺腺癌患者为研究对象,收集肺腺癌组织及其相应癌旁组织。采用实时荧光定量PCR(RT-PCR)法检测癌组织及癌旁组织中PTGS2 mRNA表达水平;采用免疫组织化学法分析PTGS2蛋白表达;采用RT-PCR法检测癌组织及癌旁组织EGFR基因突变情况。分析肺腺癌患者临床病理参数与PTGS2 mRNA水平、EGFR基因突变情况的关系。采用Spearman秩相关分析探讨肺腺癌患者EGFR基因突变情况与PTGS2 mRNA水平的相关性。结果 57例肺腺癌患者中,5例癌旁组织EGFR基因突变型患者,其对应癌组织也均发生突变,且为同一突变类型。26例(45.61%)癌组织EGFR基因突变型患者,未发现双重突变,其中19外显子突变17例(29.82%),均为缺失突变;21外显子突变9例(15.79%),均为L858R点突变。癌组织中PTGS2mRNA表达水平、PTGS2蛋白阳性率及EGFR基因突变型比例高于癌旁组织,EGFR基因野生型比例低于癌旁组织(P<0.01)。肺腺癌患者性别、TNM分期、吸烟与PTGS2 mRNA表达水平、EGFR基因突变情况有关(P<0.05或0.01)。EGFR基因突变型PTGS2 mRNA表达水平高于EGFR基因野生型(P<0.01)。肺腺癌患者EGFR基因突变与PTGS2 mRNA表达水平呈正相关(r=0.512,P<0.01)。结论 肺腺癌患者癌组织中PTGS2mRNA表达水平及PTGS2蛋白阳性率升高,且与EGFR基因突变关系密切,二者可能共同影响疾病进程。Objective To analyze the correlation between prostaglandin-endoperoxide synthase 2(PTGS2)expression level in lung adenocarcinoma tissue and epidermal growth factor receptor(EGFR)gene mutation.Methods Fifty-seven patients with lung adenocarcinoma were selected as research objects and lung adenocarcinoma tissues and their corresponding paracancer tissues collected.PTGS2 mRNA expression levels in cancer and paracancer tissues were detected adopting real-time polymerase chain reaction(RT-PCR),PTGS2 expression levels were analyzed adopting immunohistochemistry.EGFR gene mutations in cancer and paracancer tissues were detected adopting RT-PCR method.The relationships of clinicopathologic parameters with PTGS2 mRNA level and EGFR gene mutation were analyzed.Spearman rank correlation analysis was used to explore the correlation between EGFR gene mutations and PTGS2 mRNA levels in lung adenocarcinoma patients.Results PTGS2 mRNA expression level,the positive rate of PTGS2 protein and the proportion of EGFR gene mutation were higher and the proportion of EGFR gene wild type was lower in cancer tissue than those in paracancer tissue(P<0.01).Of 57 patients with lung adenocarcinoma,same mutations occurred in corresponding cancer tissues in 5 ones with EGFR gene mutations in paracancer tissues.Double mutations weren't found in 26(45.61%)patients with EGFR gene mutations in cancer tissues,of them 17(29.82%)developed exon 19 mutations and all of which were deletion mutations;9 cases(15.79%)developed exon 21 mutations,all of which were L858R point mutations.Gender,TNM stage and smoking were correlated with PTGS2 mRNA level and EGFR gene mutation in lung adenocarcinoma patients(P<0.05 or 0.01).PTGS2 mRNA expression level was higher in patients with EGFR gene mutation than ones with EGFR gene wild type(P<0.01).EGFR gene mutation was positively correlated with PTGS2 mRNA expression level in lung adenocarcinoma patients(r=0.512,P<0.01).Conclusion PTGS2 mRNA expression level and the positive rate of PTGS2 protein in cancer tissues of l
