奥硝唑联合改良负压封闭引流治疗口腔颌面部间隙感染患者临床效果

摘　　要：目的:探究奥硝唑与改良负压封闭引流(vacuum sealing drainage,VSD)联合治疗口腔颌面部间隙感染(oral and maxillofacial space infections,OMSI)患者临床效果及对血清白细胞计数(white blood cell count,WBC)、中性粒细胞计数(neutrophil count,NEUT)的影响。方法:回顾性分析2020年1月~2022年6月于我院接受奥硝唑联合传统切开引流术治疗的40例OMSI患者临床资料,纳入对照组;同期于我院接受奥硝唑联合改良VSD治疗的40例OMSI患者临床资料,纳入观察组。比较两组患者临床疗效(创口愈合时间、肉芽生长时间、住院时间)、术前和术后1周症状[脓肿直径、疼痛(Wong-Baker面部表情量表)、张口度]、血清因子[WBC、NEUT、降钙素原(procalcitonin,PCT)]水平,记录两组患者1周内并发症(感染、败血症、纵膈炎、心力衰竭)发生情况。结果:观察组患者住院时间、创口愈合时间、肉芽生长时间均短于对照组(P<0.05);术后1周,两组患者脓肿直径、Wong-Baker评分、WBC、NEUT、PCT水平均较术前降低,且观察组低于对照组(P<0.05),而张口度较治疗前增大,观察组大于对照组(P<0.05);术后1周内,观察组并发症发生率低于对照组(P<0.05)。结论:奥硝唑联合改良VSD治疗OMSI效果显著,可改善患者临床症状,缓解炎症反应,减少并发症发生。Objective:To explore the clinical effect of ornidazole combined with modified vacuum sealing drainage(VSD)in the treatment of oral and maxillofacial space infection(OMSI)in patients and its influence on serum white blood cell count(WBC)and neutrophil count(NEUT).Methods:The clinical data of 40 patients with OMSI who were treated with ornidazole combined with traditional incision and drainage in the hospital from January 2020 to June 2022 were retrospectively analyzed and the patients were included in control group.The clinical data of 40 patients with OMSI who received ornidazole combined with modified VSD during the same period were retrospectively collected and the patients were enrolled as observation group.The clinical efficacy(wound healing time,granulation growth time,hospital stay),and symptoms[abscess diameter,pain(Wong-Baker facial expression scale),mouth opening degree]and serum factors[WBC,NEUT,procalcitonin(PCT)]before surgery and at 1 week after surgery were compared between the two groups of patients.The complications(infection,septicemia,mediastinitis,heart failure)in the two groups were recorded within 1 week.Results:The hospital stay,wound healing time and granulation growth time in observation group were shorter than those in control group(all P<0.05).At 1 week after surgery,the abscess diameter,Wong-Baker score,WBC,NEUT and PCT in the two groups were reduced compared with those before surgery,and the above indicators in observation group were lower than those in control group,but the degree of mouth opening was increased compared with that before surgery,and the mouth opening degree was greater in observation group than that in control group(all P<0.05).Within 1 week after surgery,the incidence rates of complications in observation group were lower compared to control group(P<0.05).Conclusion:Ornidazole combined with modified VSD is effective in the treatment of OMSI,and it can improve the clinical symptoms,relieve the inflammatory response and reduce the incidence rates of complications.
