摘  要:目的 本研究基于成果导向教育(outcome based education, OBE)理念,设计了一套有肝胆专科特色的腹腔镜培训教学方案,并进行了实践验证。方法 将参加2022年度腹腔镜操作培训的40名学员随机分为试验组和对照组,每组各20名。对照组采取传统的理论授课结合实践操作培训模式;试验组学员以OBE理念为基础,采用逆向思维的方式进行课程建设,通过明确基础知识、手术技巧、综合能力3个层面的教学目标,建立多样化教学途径的教学设计完成整个教学过程。在课程结束时,采用理论知识考核和实践操作技术考核比较两组学员的培训成绩差异,并设计问卷了解学员对教学质量的评价。结果 培训后,试验组学员理论知识考核成绩,模拟器下钉板移物、抓豆子、剪圈、高低柱、穿隧道、缝合打结的实践操作考核成绩均高于对照组(P均<0.05);教学质量评价方面,试验组学员对基础知识的理解、实际操作能力、临床思维改进、课程结构设置合理度和课程整体满意度方面评分均高于对照组(P均<0.01)。结论 基于OBE理念的肝胆专科特色腹腔镜培训教学设计,为肝胆专科的教学提供了新的思路和方法,可提高腹腔镜术者实际操作能力,有助于提升医学生的专业技能和综合素质。Objective To design a laparoscopic training teaching program with the characteristics of hepatobiliary specialty based on the concept of outcome based education(OBE),and conduct its practical verification.Methods A to-tal of 40 trainees who participated in laparoscopic surgery training in 2022 were randomly divided into experimental group and control group,with 20 trainees in each.The control group received traditional theoretical teaching combined practical operation training;the experimental group of students adopted the reverse thinking approach to design the course based on the OBE concept,the entire teaching process was completed by clarifying 3 teaching objectives of basic knowledge,surgi-cal skills and comprehensive ability and establishing diversified teaching approaches in teaching design.At the end of the course,theoretical knowledge assessment and practical operation technology assessment were used to compare the differ-ences between the two groups of trainees,and the questionnaire was designed to explore the trainees'evaluation of teach-ing quality.Results After the training,the theoretical knowledge assessment scores of the students in experimental group,as well as the practical operation assessment scores of nail board moving,grabbing beans,cutting circles,high low column,tunnel,sewing and knotting under the simulator were higher than those of the control group(all P<0.05);as for the teaching quality evaluation,the scores of basic knowledge understanding,practical operation ability,clinical thinking improvement,course structure setting and overall course satisfaction in two groups of trainees were higher than the control group.(all P<0.05).Conclusion Based on the OBE concept,the design of laparoscopic training and teach-ing for hepatobiliary specialty provides new ideas and methods for the teaching of hepatobiliary specialty,which can im-prove the practical operational ability of laparoscopic surgeons and help enhance the professional skills and comprehensive quality of medical students.
