调神益智汤联合生物反馈对学龄期多动症患儿血清ACTH、CORT水平的影响

摘　　要：目的:探究调神益智汤联合生物反馈对学龄期多动症患儿(脾虚肝亢证)注意力集中程度、血清促肾上腺皮质激素(ACTH)、皮质醇(CORT)水平的影响。方法:选取2020年1月~2021年12月本院就诊140例学龄期多动症患儿(脾虚肝亢证)作为观察对象,采用简单随机化法分为观察组和对照组,每组各70例。对照组采用生物反馈进行治疗,观察组在其基础上联合调神益智汤。比较治疗前、治疗3个月后两组中医证候积分、注意力集中情况[多动障碍筛查量表(SNAP-Ⅳ)]、血清学指标(ACTH、CORT)、认知情况[韦氏儿童智力表(C-WISC)]的变化。结果:治疗3个月后,两组中医证候积分、SNAP-Ⅳ评分均明显降低,且观察组低于对照组(P<0.05);两组血清ACTH水平、C-WISC评分均明显升高,且观察组高于对照组(P<0.05);观察组治疗有效率显著高于对照组(P<0.05)。结论:调神益智汤联合生物反馈对于学龄期多动症患儿疗效显著,可有效提高患儿注意控制,对于患儿预后有利。Objective:To explore the effect of Tiaoshen Yizhi decoction combined with biofeedback on attention concentration degree and levels of serum adrenocorticotrophic hormone(ACTH)and cortisol(CORT)in school-age children with attention deficit hyperactivity disorder(ADHD)(spleen deficiency and liver hyperactivity syndrome).Methods:140school-age children with ADHD(spleen deficiency and liver hyperactivity syndrome)admitted to the hospital from January 2020 to December 2021were selected as observation subjects,and were divided into observation group and control group by the simple randomization method,with 70cases in each group.The control group was treated with biofeedback,and the observation group was given Tiaoshen Yizhi decoction.The changes in TCM symptoms scores,attention concentration[Swanson,Nolan,and Pelham,Version Ⅳ(SNAP-Ⅳ),serological indicators(ACTH,CORT)]and cognitive status[Chinese version of Wechsler Intelligence Scale for Children(C-WISC)]were compared between the two groups before treatment and after 3months of treatment.Results:After 3months of treatment,the TCM syndromes scores and SNAP-Ⅳ scores in the two groups were significantly decreased,and the scores in observation group were lower than those in control group(P<0.05).Serum ACTH levels and C-WISC scores were significantly increased in both groups,and the levels and scores in observation group were higher than those in control group(P<0.05).The effective rate of treatment in observation group was higher than that in control group(P<0.05).Conclusion:Tiaoshen Yizhi decoction combined with biofeedback has a significant efficacy on school-age children with ADHD,and it can effectively improve the attention control and is beneficial to the prognosis of children.
