摘  要:目的观察以问题为基础的教学法(problem-based learning,PBL)联合以案例为基础的教学法(case-based learning,CBL)在全科医学专业住院医师规范化培训(简称住培)中的应用效果。方法选取2019年9月-2021年9月在大理白族自治州人民医院参加住培的全科医学专业住院医师66名,随机分成2组,观察组33名,对照组33名,其中观察组采取CBL联合PBL教学法,对照组采取传统多媒体教学法,比较2组住院医师理论知识成绩及临床操作技能成绩,课后通过问卷调查住院医师对教学模式的认可程度。结果观察组理论知识成绩为(42.30±3.82)分,高于对照组的(41.58±3.65)分,但差异无统计学意义(P>0.05),观察组病例考核评分为(25.91±2.14)分,技能操作考核评分为(16.85±1.50)分,均高于对照组(23.39±2.50)分和(14.64±2.41)分,差异有统计学意义(P<0.001);观察组住院医师对教学方式中学习兴趣、自主能力、解决能力、协作能力、沟通能力提高的认可程度评分均高于对照组,差异具有统计学意义(P<0.001)。结论在全科医学专业住培工作中应用PBL联合CBL教学法教学效果显著,住院医师对该教学方法认可度较高。Objective To observe the application effect of problem-based learning(PBL)combined with case-based learning(CBL)teaching model in standardized resident training of general practice.Methods A total of 66 general practitioners in the in Dali Bai Autonomous Prefecture People's Hospital from September 2019 to September 2021 were randomly divided into two groups,33 resident physicians in the observation group and 33 resident physicians in the control group.The observation group adopted the PBL combined with CBL teaching mode,and the control group adopted the traditional teaching mode.The theoretical scores and clinical skills scores of the two groups of residents were compared.Moreover,students'recognition of the teaching mode was investigated through a questionnaire star.Results Compared with the control group,the scores of theoretical knowledge in the observation group were(42.30±3.82)points,higher than(41.58±3.65)points in the control group,but the difference was not statistically significant(P>0.05).The scores of case analysis points and skill operation assessment points in the observation group were(25.91±2.14)points and(16.85±1.50)points,higher than(23.39±2.50)points and(14.64±2.41)points of the control group,with statistically significant differences(P<0.001).The recognition scores of the observation group's resident physicians for the improvement of learning interest,autonomy,problem-solving ability,collaboration ability,and communication ability in teaching methods were higher than those of the control group's resident physicians,and the difference was statistically significant(P<0.001).Conclusion In standardized resident training of general practice,the teaching model of PBL combined with CBL has a remarkable effect,and resident physicians have a high recognition of this teaching method.
