摘  要:目的探讨以问题为基础的教学法(problem-based learning,PBL)在心内科住院医师规范化培训中的应用效果。方法选择2020年9月-2022年7月在郑州大学第一附属医院心内科接受培训的100名住培医师,采用随机数字表法分为PBL组和对照组,每组50名。PBL组采用PBL教学法,对照组采用传统授课教学法。采用出科考核及学习积极主动性量表评估2组住培医师的教学成果。结果PBL组现病史、家族史询问不合格率低于对照组(P<0.05);PBL组视诊、触诊、叩诊、听诊不合格率均低于对照组(P<0.05);胸腔穿刺考核中PBL组准备工作、穿刺结果不合格率均低于对照组(P<0.05),差异有统计学意义。PBL组学习积极主动性评分总分为(80.04±12.40)分,高于对照组的(62.50±5.81)分(P<0.05),差异具有统计学意义。结论在心内科住院医师规范化培训中应用PBL教学法有助于提高住培医师的临床实践能力和主观能动性。Objective To explore the application effect of PBL teaching method in standardized training of cardiology resident physicians.Methods A total of 100 residents trained in the department of cardiology of the First Affiliated Hospital of Zhengzhou University from September 2020 to July 2022 were selected.These residents were divided into an intervention group or a control group by random number table,with 50 residents in each group.The intervention group was taught with PBL teaching method,while the control group was taught with traditional teaching method.A graduation assessment exam was implemented,and a learning initiative scale was used to evaluate the teaching outcomes of the two groups of residents.Results The unqualified rate of current medical history and family history inquiry in the PBL group was lower than those in the control group(P<0.05);The failure rates of visual examination,palpation,percussion,and auscultation in the PBL group were lower than those in the control group(P<0.05).The preparation work and failure rate of puncture results in the PBL group were lower than those in the control group in the thoracic puncture assessment(P<0.05),the difference between the two groups was statistically significant.The total scores of learning initiative in the PBL group were(80.04±12.40)points,significantly higher than(62.50±5.81)points in the control group,the difference between the two groups was statistically significant(P<0.05).Conclusion In the standardized training of resident physicians in cardiology,the PBL teaching method can improve the clinical practice ability and subjective initiative of the trained physicians.
