摘　　要：目的分析白介素-6(IL-6)、白介素-10(IL-10)、白细胞计数/淋巴细胞比值(WBC/LYM)在口腔颌面部间隙感染患者中的表达水平差异及诊断价值。方法选取2019年1月至2022年3月因口腔颌面部间隙感染(OMSI)就诊于徐州医科大学附属医院的144例患者为病例组。选取同期无炎症的腮腺良性肿瘤患者136例为对照组,收集两组患者治疗前IL-6、IL-10、WBC/LYM、C-反应蛋白(CRP)、中性粒细胞百分比(NEU%)、白细胞计数(WBC),采用单因素分析和受试者工作特征(ROC)曲线分析IL-6、IL-10、WBC/LYM与OMSI患者临床特征的关系及诊断价值。结果病例组IL-6、IL-10、WBC/LYM水平较对照组增高,差异有统计学意义(P<0.05);ROC曲线结果显示,IL-10、IL-6、WBC/LYM的曲线下面积(AUC)分别为0.868、0.947和0.942,5种因子联合检测的AUC为0.985,灵敏度为0.962,特异度为0.948。结论IL-10、IL-6、WBC/LYM可联合CRP、WBC和NEU%高效准确地诊断OMSI,高水平IL-6、IL-10、WBC/LYM是OMSI患者住院时间延长、发生并发症和术后需要ICU监护的重要参考指标。Objective To analyze the expression level difference and diagnostic efficacy of interleukin-6(IL-6),interleukin-10(IL-10)and white blood cell count/lymphocyte ratio(WBC/LYM)in patients with oral and maxillofacial space infection.Methods We selected the data of 144 patients who were admitted to the Affiliated Hospital of Xuzhou Medical University due to oral and maxillofacial space infection(OMSI)from January 2019 to March 2022.At the same time,136 patients with benign parotid tumors without inflammation were selected as the control group.IL-6,IL-10,WBC/LYM,C-reactive protein(CRP),percentage of neutrophils(NEU%),white blood cell count(WBC)were col‑lected before treatment,and the relationship between IL-6,IL-10,WBC/LYM and the clinical characteristics of OMSI patients and diagnostic value was analyzed using single factor analysis and ROC curve.Results The levels of IL-6,IL-10 and WBC/LYM in OMSI patients were sig‑nificantly higher than those in non space infection patients,and the difference was statistically significant(P<0.05);the ROC curve results showed that the area under curve(AUC)of IL-10,IL-6,and WBC/LYM was 0.868,0.947,and 0.942,respectively.The AUC for combined de‑tection of five factors was 0.985,the sensitivity was 0.962,and the specificity was 0.948.Conclusion IL-10,IL-6,WBC/LYM can be com‑bined with CRP,WBC and NEU%to diagnose OMSI efficiently and accurately.High levels of IL-6,IL-10,WBC/LYM are important reference indicators for OMSI patients to prolong hospital stay,develop complications and require ICU monitoring after surgery.
