摘  要:目的 评估玻璃体视网膜手术模拟器联合Wetlab教学模式在眼科住院医师微创玻璃体手术(minimally invasive vitreous surgery,MIVS)培训中的效果。方法 选择2022年1月-2022年12月在深圳市眼科医院眼底病科轮转的住院医师20名,按照培训方式不同分为研究组与对照组,研究组接受玻璃体视网膜手术模拟器联合Wetlab教学模式培训,对照组接受Wetlab教学模式培训,分析2组教学效果。结果 研究组经过玻璃体视网膜手术模拟器训练后导航模块、镊子夹持模块、防抖模块、双手操作模块、玻璃体后脱离模块及玻璃体切割模块的考核评分分别为(88.60±3.26)分、(90.67±2.91)分、(89.96±2.87)分、(88.06±2.74)分、(86.01±2.90)分及(84.59±2.81)分,均高于训练前,差异有统计学意义(P <0.001);研究组在Wetlab教学模式下的导光照明、双手操作协调性、双手操作稳定性、中轴部玻璃体切割及玻璃体后脱离的考核评分分别为(5.12±1.13)分(、4.31±0.35)分(、4.23±1.14)分、(4.29±0.48)分及(3.62±1.24)分,均高于对照组,差异有统计学意义(P <0.05);研究组对培训方式的新颖性、培训贴近真实手术体验、对玻璃体视网膜手术技能的帮助、有信心进行真实手术及手术培训的整体满意度评价得分分别为(4.64±0.12)分、(4.18±0.17)分、(4.42±0.33)分、(4.18±0.82)分及(4.75±0.12)分,均高于对照组,差异有统计学意义(P <0.05)。结论 玻璃体视网膜手术模拟器联合Wetlab教学模式能有效提升眼科住院医师的MIVS培训效果及住院医师对教学的满意度。Objective To evaluate the application effect of vitreoretinal surgery simulator combined with Wetlab teaching mode in minimally invasive vitreous surgery(MIVS)training for ophthalmic residents.Methods A total of 20 residents in the department of ocular fundus disease,Shenzhen Eye Hospital in 2022 were divided into research group and control group.The research group adopted vitreoretinal surgery simulator combined with Wetlab teaching mode training and the control group adopted Wetlab teaching mode training.The training effects of the two groups were analyzed.Results The test scores of navigation module,forceps module,anti-shaking module,two-handed operation module,post-glass detachment module and vitreous cutting module of the research group after the training of vitreoretinal surgery simulator were(88.60±3.26)points,(90.67±2.91)points,(89.96±2.87)points,(88.06±2.74)points,(86.01±2.90)points and(84.59±2.81)points,which were higher than before training,and the differences were all statistically significant(P<0.001).In the Wetlab teaching mode,the test scores of guiding light,coordination of both hands,stability of both hands,vitreous incision of central axis and post-vitreous detachment of the research group were(5.12±1.13)points,(4.31±0.35)points,(4.23±1.14)points,(4.29±0.48)points and(3.62±1.24)points,which were all higher than those of the control group,and the differences were statistically significant(P<0.05).The evaluation scores of novelty of training method,close to real surgical experience,helpfulness of vitreoretinal surgery skills,confidence in real surgery and overall satisfaction with surgical training of the research group were(4.64±0.12)points,(4.18±0.17)points,(4.42±0.33)points,(4.18±0.82)points and(4.75±0.12)points,which were higher than those of the control group,the differences were all statistically significant(P<0.05).Conclusion Vitreoretinal surgical simulator combined with Wetlab teaching mode can effectively improve the MIVS training effect and the satisfaction with teaching
