摘 要:目的探讨典型病例结合问题式学习(PBL)模式对肛肠科规范化培训(以下简称“规培”)研究生临床教学的应用效果。方法选取进入肛肠科接受规培的专业型硕士研究生120名作为研究对象,根据专业是否为肛肠方向进行分层,然后再分为两组,各60名。其中肛肠方向每组20名,非肛肠方向每组40名,性别不限。对照组采用传统教学模式,实验组采用典型病例结合PBL教学模式。分别对两组学生采用考试和问卷调查相结合的方式进行教学效果评价。结果理论考试及病例分析考核成绩比较,无论是肛肠方向还是非肛肠方向,实验组成绩均优于对照组(P<0.05);且实验组肛肠方向优于非肛肠方向(P<0.05)。学习兴趣、注意力集中、记忆归纳能力、学习内容理解程度4个小项满意度比较,实验组分数均高于对照组(P<0.05),且观察组肛肠方向分数高于非肛肠方向(P<0.05)。结论典型病例结合PBL教学模式有助于提高肛肠科危急重症的临床教学效果,并与专业方向存在相关性。Objective To investigate the effect of using typical cases combined with the PBL teaching mode in clinical teaching of post-graduate students in proctology department.Methods One hundred and twenty professional master’s degree students who had entered the regulation stage of anorectal medicine were selected as the study subjects,stratified according to whether they were in the anorectal direction or not,and then each group was further divided into 2 groups,20 each in the anorectal direction and 40 each in the non-anorectal direction,with no restriction on gender.The control group adopted the traditional teaching mode,and the experimental group adopted the typical case combined with PBL teaching mode.A combination of examinations and questionnaires were used to evaluate the teaching effects of the four groups of students respectively.Results The results of theoretical examination and case analysis were better than those of control group in anorectal direction or non-anorectal direction(P<0.05).The anorectal direction of the experimental group was better than that of the non-anorectal direction(P<0.05).Compared with the four subitems of satisfaction,namely,learn-ing interest,concentration,memory and induction ability,and understanding degree of learning content,the experimental group had higher scores than the control group(P<0.05),and the anorectal direction of the observation group was higher than that of the non-anorectal direction(P<0.05).Conclusion Typical cases combined with PBL teaching mode can help improve the clinical teaching effect of critical care in anorectal medicine,and there is a correlation with the specialty direction.
