双相情感障碍患者病耻抵抗现况及其影响因素

摘　　要：目的 调查双相情感障碍(BD)患者病耻抵抗情况,进一步分析其影响因素,为临床及时采取针对性措施提升患者病耻抵抗水平奠定基础。方法 选取2020年6月至2023年6月商丘市第二人民医院精神科收治的237例BD患者,采用病耻抵抗量表(SRS)调查研究对象病耻抵抗情况,并采用问卷收集其一般资料及其他可能影响SRS评分的指标,多元线性回归分析BD患者病耻抵抗的影响因素。结果 237例BD患者SRS总分为(9.59±2.51)分。病耻抵抗低水平患者杨氏躁狂评定量表(YMRS)评分[(32.10±3.68)分]>中等水平患者[(21.65±4.24)分]>高等水平患者[(11.34±2.98)分],差异有统计学意义(P<0.05),且汉密尔顿抑郁量表(HAMD)评分[(38.67±4.51)分]>中等水平患者[(26.57±3.69)分]>高等水平患者[(14.25±4.18)分],差异有统计学意义(P<0.05)。相关性分析显示,社会支持评定量表(SSRS)、自知力与治疗态度问卷(ITAQ)、个人和社会功能量表(PSP)评分与SRS评分呈正相关(r=0.465、0.684、0.715,P<0.001),面子观量表(CSF)评分与SRS评分呈负相关(r=-0.565,P<0.001)。多元线性回归分析显示,与他人共同居住、CSF评分、家庭经济月收入、PSP评分、SSRS评分、ITAQ评分均是BD患者病耻抵抗的影响因素(P<0.05)。结论 BD患者病耻抵抗受社会支持、面子观、自知力等多种因素影响,处于中等水平,临床可依据以上分析施以针对性干预以提升患者病耻抵抗水平。Objective To investigate the status of stigma resistance in patients with bipolar disorder(BD)and further analyze its influencing factors,so as to lay a foundation for taking timely targeted measures to improve the level of stigma resistance in patients.Methods A total of 237 BD patients from Department of General Psychiatry,Shangqiu Second People’s Hospital from June 2020 to June 2023 were selected.The stigma resistance scale(SRS)was used to investigate the stigmatization resistance of the subjects,and the general information and other indicators that may affect the SRS score were collected by questionnaires.The influencing factors of stigma resistance in BD patients were analyzed by multiple linear regression.Results The total SRS score of 237 BD patients was(9.59±2.51)points.The score of Young’s mania rating scale(YMRS)in patients with low level of stigma resistance(32.10±3.68)>in patients with intermediate level(21.65±4.24)>in patients with advanced level(11.34±2.98),and the difference was statistically significant(P<0.05).The score of Hamilton depression scale(HAMD)in low patients(38.67±4.51)>intermediate patients(26.57±3.69)>advanced patients(14.25±4.18),and the difference was statistically significant(P<0.05).Correlation analysis showed that the scores of social support rating scale(SSRS),insight and treatment attitudes questionnaires(ITAQ)and personal and social performance scale(PSP)were positively correlated with SRS scores(r=0.465、0.684、0.715,P<0.001).Face perception scale(CSF)score was negatively correlated with SRS score(r=-0.565,P<0.001).Multiple linear regression analysis showed that living with others,CSF score,monthly family economic income,PSP score,SSRS score and ITAQ score were all influencing factors of stigma resistance in BD patients(P<0.05).Conclusion The stigma resistance of BD patients is affected by social support,face view,self-knowledge and other factors,and is at a moderate level.Clinical intervention can be carried out according to the above analysis to enhance the le
