摘 要:目的分析儿童及青少年甲状腺癌患者的超声声像图特征,探讨超声对青少年甲状腺癌的诊断价值。方法回顾性分析2018年1月至2023年11月在郑州大学第一附属医院手术的18岁及以下的154例甲状腺癌患者的临床资料,以病理结果为诊断标准,分析其超声声像图特征。结果154例儿童及青少年甲状腺癌患者中4例呈弥漫型改变:甲状腺双侧叶及峡部未见具体结节,可见多发点状强回声弥漫簇状分布,呈"暴风雪"征;150例儿童及青少年甲状腺癌患者呈结节样病灶,边界不清112例(74.7%),形态不规则97例(64.7%),纵横比>1者41例(27.3%),伴钙化118例(78.6%),甲状腺被膜侵犯39例(25.3%),颈部异常肿大淋巴结106例(68.8%),分子病理结果示鼠类肉瘤滤过性毒菌致癌同源体B1(V600E)阳性65例(42.2%)。结论儿童及青少年甲状腺癌具有典型的超声声像图特征,高频彩色多普勒超声检查可作为儿童及青少年甲状腺癌诊断及鉴别诊断的首选检查方法。Objective To analyze the sonographic features of thyroid cancer in children and adolescents,and to investigate the diagnostic value of ultrasound for thyroid cancer in children and adolescents.Methods The clinical data of 154 thyroid cancer patients aged 18 years or below who were treated by surgery in the First Affiliated Hospital of Zhengzhou University from January 2018 to November 2023 were retrospectively analyzed.Taking pathological results as diagnostic criteria,the sonographic features of the selected patients were analyzed.Results Out of 154 children and adolescents with thyroid cancer,4 cases showed diffuse lesions;no specific nodule was found in the bilateral lobes or isthmus of the thyroid gland;multiple clustered diffuse punctate echogenic foci were seen,with blizzard signs.Another 150 children and adolescents with thyroid cancer showed nodular lesions,manifesting unclear borders in 112 cases(74.7%),irregular morphology in 97 cases(64.7%),aspect ratio>1 in 41 cases(27.3%),calcification in 118 cases(78.6%),invasion of the thyroid peritoneum in 39 cases(25.3%),abnormally enlarged lymph nodes in the neck in 106 cases(68.8%),and molecular pathology findings of v-Raf murine sarcoma viral oncogene homolog B1(V600E)positive in 65 cases(42.2%).Conclusions Thyroid cancer in children and adolescents has typical sonographic features.And high-frequency colour Doppler can be used as the preferred examination method for the diagnosis and differential diagnosis of thyroid cancer in children and adolescents.
