超声内镜引导下食管异物取出1例报道

摘　　要：本文报道1例60岁女性患者进食鱼肉后出现咽部异物感,持续3周,曾2次行喉镜检查,未见异常。行胸部CT检查提示食管异物。给予抗生素保守治疗后效果欠佳。后收至我院消化内科住院治疗,行胃镜检查未发现异物,但发现食管上段糜烂、隆起,超声内镜检查提示异物嵌入食管壁,后在超声内镜引导下行食管黏膜切开,暴露异物,异物钳夹持取出异物。给予补液、营养支持、抗感染等治疗措施后,患者顺利出院。This article reported a 60 years old woman who developed foreign body sensation in her throat after eating fish for 3 weeks.Laryngoscopy was performed twice and no abnormality was found.However,CT examination of the chest revealed a foreign body in the esophagus.Conservative treatment with antibiotics yielded poor results.She was later admitted to the Department of Gastroenterology for hospitalization.No foreign body was found during gastroscopy,but erosion and bulges were found in the upper esophagus.Endoscopic ultrasound showed that the foreign body was embedded in the esophageal wall.The esophageal mucosa was then incised under the guidance of endoscopic ultrasound to expose the foreign body,and the foreign body was extracted by forceps.After being given rehydration,nutritional support,anti-infection and other treatment measures,the patient was discharged successfully.
