摘　　要：目的研究血清淀粉酶(AMY)、脂肪酶(LPS)及C反应蛋白(CRP)联合检验在急性胰腺炎(AP)诊断中的应用价值。方法以2021年6月-2023年6月万载县中医院收治的100例AP患者为研究对象,设为AP组,其中轻症AP患者68例,重症AP患者32例。另选取同期80例体检健康者为对照组,两组均检测AMY、LPS、CRP,比较AP组与对照组、轻症AP患者与重症AP患者的AMY、LPS、CRP检测结果,分析以上指标及其联合检验对AP疾病的诊断价值。结果AP组AMY、LPS、CRP水平高于对照组(P<0.05)。重症AP患者的CRP高于轻症AP患者(P<0.05),但两组AMY、LPS水平比较,差异无统计学意义(P>0.05)。经ROC曲线分析显示,AMY、LPS、CRP检测对AP均具有确切诊断价值(AUC>0.5),且三者联合检验AUC值高于其单一检验(P<0.05)。结论AMY、LPS、CRP在AP诊断中均具有确切诊断价值,其联合检验诊断效能最佳。Objective To study the application value of combined detection of serum amylase(AMY),lipase(LPS)and C-reactive protein(CRP)in the diagnosis of acute pancreatitis(AP).Methods A total of 100 patients with AP admitted to Wanzai County Hospital of Traditional Chinese Medicine from June 2021 to June 2023 were selected as the AP group,including 68 patients with mild AP and 32 patients with severe AP.In addition,80 healthy subjects were selected as the control group.AMY,LPS and CRP were detected in both groups.The results of AMY,LPS and CRP in AP group and control group,mild AP patients and severe AP patients were compared.The diagnostic value of the above indexes and their combined tests for AP disease was analyzed.Results The levels of AMY,LPS and CRP in AP group were higher than those in control group(P<0.05).The CRP of severe AP patients was higher than that of mild AP patients(P<0.05),but there was no significant difference in AMY and LPS levels between the two groups(P>0.05).According to ROC curve analysis,AMY,LPS and CRP had definite diagnostic value for AP(AUC>0.5),and the AUC value of the combined test was higher than that of the single test(P<0.05).Conclusion AMY,LPS and CRP have definite diagnostic value in the diagnosis of AP,and the combined test has the best diagnostic efficiency.
