摘 要:目的通过与常规教学模式对比,探索支气管模拟器(ORSIM)在胸科麻醉双腔支气管插管教学中的应用价值。方法选取空军军医大学唐都医院麻醉科48名低年资住院医师为主要研究对象,随机将所有住院医师分为采用常规教学模式的CON组(n=24)和使用ORSIM的SIM组(n=24)。使用对应方式进行教学,然后对比组间支气管插管评价指标和研究对象对各自教学模式和内容的满意程度。结果教学后CON组和SIM组的支气管插管综合得分(73.9±10.3 vs.82.4±8.2;P<0.001)、耗费时间[(150.4±11.5)s vs.(137.5±14.1)s,P=0.011]和总体满意度(70.8%vs.95.8%,P=0.022),SIM组均优于CON组;插管成功率对比差异无统计学意义(50.0%vs.70.8%,P=0.140)。结论使用ORSIM对低年资麻醉科住院医师进行短期双腔支气管插管教学,可以缩短插管时间、提升插管质量和总体满意度,其在胸科麻醉的气道管理教学中具有应用价值。Objective To explore the application value of ORSIM bronchial simulator in the teaching of dual-lumen bronchial intubation in thoracic anesthesia by comparing it with the conventional teaching mode.Methods A total of 48 junior anesthesia residents from the Department of Anesthesiology,Tangdu Hospital,Air Force Medical University were selected as the research subjects and randomly divided into the COM group in which the conventional teaching mode was applied and the SIM group in which ORSIM was used,with 24 subjects in each.The teaching was conducted using the corresponding mode,and the evaluation indexes of bronchial intubation of each group and the residents’satisfaction with the respective training mode and content were compared.Results After completing the training,the comprehensive score of bronchial intubation of the CON group and the SIM group was(73.9±10.3 vs.82.4±8.2;P<0.001),time consumption of the CON group and the SIM group was[(150.4±11.5)s vs.(137.5±14.1)s;P=0.011]and the overall satisfaction of the CON group and the SIM group was(70.8%vs.95.8%;P=0.022).The SIM group performed significantly better than the CON group,but there was no significant difference in the success rate of intubation between the two groups(50.0%vs.70.8%;P=0.140).Conclusion The application of ORSIM in the short-term teaching of dual-lumen bronchial intubation of junior anesthesia residents can shorten the intubation time,improve the quality and satisfaction of bronchial intubation.It is of good application value in the air tube management teaching of thoracic anesthesia.
