摘  要:住院医师规范化培训(住培)是我国医药卫生体制改革和深化医学教育改革的重大举措,是医学生毕业后继续教育的核心部分,在医学生向临床医师转变进程中扮演着承前启后的重要角色。超声医学专业住培医师不仅要掌握超声理论知识与操作技能,更应具备良好的临床思维,而临床思维的培养是超声科住培工作的重点与难点。传统教学模式在培养临床思维方面存在耗时久、效率低下的弊端。鉴于此,笔者尝试将混合教学法与阶梯式培养策略融合,引入到超声科住院医师规范化培训(住培)中,旨在对传统的临床思维培养教学方式进行优化与改进。通过混合教学法联合阶梯式培养策略:住培早期阶段,住培医师可以在导师的悉心指导下,循序渐进地学习超声专业知识,初步构建起临床思维的基本认知架构;住培中晚期阶段,住培医师通过临床实践持续反复地进行逻辑推理与假设验证的练习,在此过程中察觉到自身知识与思维的漏洞并及时弥补,有力地突破固有思维模式的桎梏,从而实现临床思维能力的稳步提升与持续优化,为其日后的临床工作奠定坚实基础,也为超声科住培教学改革提供了有益的实践探索范例。The system of standardized training for residents(referred to as“standardized training”)is a major initiative for the reform of China’s medical health system as well as medical education reform.As an important part of postgraduate continuing education for medical students,it plays an important role in the transition of medical students into clinicians.The residents of ultrasound medicine should not only master the theoretical knowledge and operational skills of ultrasound,but also have good clinical thinking skill,and the training of clinical thinking is the focus and difficulty of the standardized training work of ultrasound department.Traditional teaching requires a long period of clinical practice to effectively train residents’clinical thinking,which is time-consuming and inefficient.Therefore,the author tried to combing the blended teaching method and step-wise training strategy,and introduce it into the standardized training of ultrasound residents,aiming to optimize and improve the traditional clinical thinking training teaching method.By using the blended teaching method combined with step-wise training strategy:in the early stage of standardized training,residents can gradually master the professional knowledge of ultrasound under the careful guidance of mentors,and initially construct the basic cognitive framework of clinical thinking;in the middle and late stages of standardized training,residents repeatedly carry out the practice of logical reasoning and hypothesis verification through clinical practice,they can be aware of the loopholes in their knowledge and thinking and make up for them in time,effectively breaking through the shackles of the inherent thinking mode,so as to continuously improve the level of clinical thinking and gradually build and optimize clinical thinking.It lays a solid foundation for their future clinical work and also provides a beneficial practical exploration example for the teaching reform of standardized training in the ultrasound department.
