元认知指导下PBL在重症医学科中医住院医师规范化培训中的应用研究

摘  要:目的分析在重症医学科住院医师规范化培训中应用元认知指导下以问题为基础的学习(problembased learning,PBL)教学法的效果。方法选取2019年12月—2021年8月在广西中医药大学第一附属医院重症医学科轮转(周期为2个月)的84名住院医师规范化培训学员为研究对象。按照入科顺序分为两组,每组42名学员,教学组实施传统教学,整合组实施元认知指导下PBL教学模式。比较两组理论成绩、技能成绩、临床思维能力、综合印象评分及教学满意度。结果整合组理论成绩、技能成绩、临床思维能力、综合印象评分均优于教学组,差异有统计学意义(P均<0.05)。整合组教学总满意度为95.23%(40/42),高于教学组的76.19%(32/42),差异有统计学意义(χ^(2)=6.222,P<0.05)。结论元知识指导下PBL在重症医学科中医住院医师规范化培训中应用,能提升学员的理论知识和临床综合能力。Objective To analyze the effect of applying problem-based learning(PBL)pedagogy guided by metacogni⁃tion in the standardized training of intensive care unit residents.Methods Eighty-four trainees in standardized resi⁃dency training who rotated in the Intensive Care Unit of The First Affiliated Hospital of Guangxi University of Chinese Medicine from December 2019 to August 2021(a cycle of two months)were selected as the study subjects.They were divided into two groups according to the order of admission to the department,with forty-two trainees in each group.The teaching group implemented traditional teaching,and the integration group implemented the PBL teaching mode under the guidance of metacognition.The theory scores,skill scores,clinical thinking ability,comprehensive impres⁃sion scores and teaching satisfaction were compared between the two groups.Results The integration group´s theory scores,skill scores,clinical thinking ability,and comprehensive impression scores were better than those of the teach⁃ing group,and the differences were statistically significant(all P<0.05).Teaching satisfaction in the integration group was 95.23%(40/42),which was higher than 76.19%(32/42)in the teaching group,and the difference was statistically significant(χ^(2)=6.222,P<0.05).Conclusion Application of PBL guided by meta-knowledge in the standardized training of Chinese medicine residents in intensive care unit enhances trainees´theoretical knowledge and comprehensive clinical competence.
