摘  要:目的探讨胸腔积液细胞蜡块苏木精-伊红(HE)染色、免疫组织化学染色(IHC)技术和表皮生长因子受体(EGFR)基因检测在肺癌中的应用效果。方法选取96例疑似肺癌患者,均行胸腔积液细胞蜡块HE染色、IHC技术。分析胸腔积液细胞蜡块HE染色、IHC技术单一及联合检测对肺癌的诊断效能,并分析EGFR基因检测的临床意义。结果胸腔积液细胞蜡块HE染色结果显示,阴性17例,阳性79例;IHC技术结果显示,阴性20例,阳性76例;联合检测结果显示,阴性16例,阳性80例;病理结果显示,阴性15例,阳性81例。胸腔积液细胞蜡块HE染色、IHC技术联合检测对肺癌的诊断效能高于单一检测。81例肺癌患者IHC结果显示,EGFR阳性表达36例,强阳性21例,弱阳性15例;IHC技术EGFR弱阳性患者中,基因检测EGFR阳性11例。结论胸腔积液细胞蜡块HE染色联合IHC技术对肺癌的诊断效能较高,检测EGFR表达有助于指导临床治疗。Objective To explore the application of hematoxylin-eosin(HE)staining,immunohistochemical(IHC)staining and epidermal growth factor receptor(EGFR)gene detection of wax block of pleural effusion cells in lung can-cer.Method A total of 96 patients with suspected lung cancer were selected and treated with HE staining of wax block and IHC.The diagnostic efficacy of wax blocks of pleural effusion cells with HE staining and IHC single and combined detection in lung cancer,as well as the clinical significance of EGFR expression were analyzed.Result The results of wax block of pleural effusion cells with HE staining showed that 17 cases were negative and 79 cases were positive.The results of IHC showed that 20 cases were negative and 76 cases were positive.Combined test results showed that 16 cases were negative and 80 cases were positive.The pathological results showed that 15 cases were negative and 81 cases were positive.The combined detection of wax block of pleural effusion with HE staining and IHC in the diagnosis of lung can-cer was more effective than single detection.Among the 81 positive patients,36 cases showed positive expression of EG-FR by IHC,including 21 cases strong positive and 15 cases weak positive,11 cases weak positive patients by IHC were positive by EGFR gene test.Conclusion Wax block of pleural effusion of HE staining combined with IHC is effective in the diagnosis of lung cancer,and can broaden the application range of EGFR gene mutation detection,and the detection of EGFR expression is helpful for guiding clinical treatment.
