慈善医疗救助对广东省河源市白血病患儿异地就医与直接非医疗经济负担关系的影响

摘　　要：目的:探讨慈善医疗救助在广东省河源市白血病患儿异地就医与直接非医疗经济负担间的作用机制,为完善儿童重大疾病慈善医疗救助模式提供针对性建议。方法:采用整群抽样法选取85名在2017年8月-2022年6月期间参与“联爱工程”慈善医疗救助项目的广东省河源市白血病患儿及家庭作为干预组,采用滚雪球抽样法选取36名未接受项目救助的患儿作为对照组。使用SPSS 26.0软件的PROCESS 3.3宏程序,分析慈善医疗救助在白血病患儿异地就医与直接非医疗经济负担之间的调节作用。结果:本地就医和异地就医白血病患儿直接非医疗经济负担分别为7.92万元和8.46万元。白血病患儿异地就医与直接非医疗经济负担呈正相关关系(r=0.193,P<0.05)。异地就医对白血病患儿直接非医疗经济负担有正向作用(β=19.610,P<0.05),慈善医疗救助对直接非医疗经济负担有负向作用(β=-26.415,P<0.05),且慈善医疗救助在白血病患儿异地就医对直接非医疗经济负担的影响中起负向调节作用(β=-13.813,P<0.05)。结论:白血病患儿直接非医疗经济负担沉重,异地就医与直接非医疗经济负担有明显的正相关性。慈善医疗救助在白血病患儿异地就医对直接非医疗经济负担的影响中发挥着调节作用。慈善医疗救助通过其人文关怀和医疗能力提升等干预活动,能够有效地减轻白血病患儿异地就医所带来的直接非医疗经济负担的增长。Objective:To explore the mechanism of charitable medical assistance in the relationship between medical treatment in non-resident place and the direct non-medical financial burden among pediatric leukemia patients in Heyuan city of Guangdong province,and to guide the improvement of the charitable medical assistance mode for children with major diseases in China.Methods:A cluster sampling method was employed to select 85 pediatric leukemia patients and their caregivers who participated in the"United Love Program"as the intervention group from August 2017 to June 2022 in Heyuan city of Guangdong province.Additionally,a snowball sampling method was used to adopt 36 patients who did not receive project assistance as the control group.The moderating effect of charitable medical assistance between medical treatment in non-resident place and direct non-medical financial burden among leukemia patients was analyzed using the PROCESS 3.3 macro program in SPSS 26.0.Results:The direct non-medical financial burden for pediatric leukemia patients receiving treatment locally and in a different location was 79200 yuan and 84600 yuan.There was a positive correlation between medical treatment in non-resident place and the direct non-medical financial burden(r=0.193,P<0.05).The medical treatment in non-resident place positively predicted the direct non-medical financial burden for pediatric leukemia patients(β=19.610,P<0.05).Charitable medical assistance had a negative impact on the direct non-medical financial burden(β=-26.415,P<0.05).Moreover,charitable medical assistance played a negative moderating role in the impact of medical treatment in non-resident place on the direct non-medical financial burden for pediatric leukemia patients(β=-13.813,P<0.05).Conclusion:Families of pediatric leukemia patients bear a heavy direct non-medical financial burden.There is a significant positive correlation between medical treatment in non-resident place and direct non-medical financial burden.Charitable medical assistance plays a moderatin
