度洛西汀联合喹硫平治疗青少年抑郁症的疗效及对精神状态和睡眠质量的影响

摘　　要：目的 观察度洛西汀联合喹硫平治疗青少年抑郁症的疗效及对精神状态和睡眠质量的影响。方法 选择2020年8月—2023年7月于福建省福州神经精神病防治院接受治疗的青少年抑郁症患者80例,采用抓阄法随机分为联合组和对照组各40例。联合组予以度洛西汀联合喹硫平治疗,对照组予以喹硫平治疗,2组均持续治疗6周。比较2组临床疗效,治疗前后精神状态、睡眠质量、血清皮质醇(Cor)、5-羟色胺(5-HT)水平及不良反应。结果 联合组总有效率高于对照组(92.50%vs. 75.00%,χ^(2)=4.501,P=0.034);治疗6周后,2组汉密尔顿抑郁量表(17项)评分、匹兹堡睡眠质量指数量表评分及血清Cor水平均降低,5-HT水平均升高,且联合组降低/升高的幅度大于对照组(P<0.01);联合组与对照组不良反应总发生率比较差异无统计学意义(17.50%vs. 10.00%,χ^(2)=0.949,P=0.330)。结论 度洛西汀联合喹硫平治疗青少年抑郁症具有较好的效果,可改善精神状态,提高睡眠质量,对血清Cor、5-HT有一定调节作用,且安全性较高。Objective To observe the efficacy of duloxetine combined with quetiapine in the treatment of adolescent depression and its effect on mental status and sleep quality.Methods Eighty cases of adolescent depression patients admitted to Fuzhou Neuropsychiatric Prevention and Treatment Hospital of Fujian from August 2020 to July 2023 were selected,and they were divided into the combined group(40 cases)and the control group(40 cases)by using the lottery method.The combined group was treated with duloxetine combined with quetiapine,and the control group was treated with quetiapine,and both groups continued treatment for 6 weeks.The clinical efficacy,mental status,sleep quality,the levels of Cor,5-HT before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the combined group was higher than that in the control group(92.50%vs.75.00%,χ^(2)=4.501,P=0.034).After 6 weeks of treatment,the HAMD-17 scores,PSQI scores,serum Cor levels decreased,and 5-HT levels increased in the two groups,and the magnitude of the reduction/increase in the combined group was greater than that in the control group(P<0.01).The difference between the total incidence of adverse reactions in the combined group and the control group was not statistically significant(17.50%vs.10.00%,χ^(2)=0.949,P=0.330).Conclusion Duloxetine combined with quetiapine has better effect in treating adolescent depression,which can improve the mental status,improve the quality of sleep,and have a certain modulation effect on serum Cor,5-HT,and higher safety.
