摘 要:法医学医疗损害鉴定中揭示漏诊误诊、漏治误治、未尽充分注意义务是医疗过错的常见三方面情况,主要原因是临床医生缺乏整合医学知识结构和整体医学思维模式。迄今,尚未见行之有效的整合医学和整体医学的医学教育与继续教育方法。基于法医鉴定的取证-循证-论证三大核心工作的证据优势,参照《国际疾病分类》的结构化医学死亡证明的思维模式,每一医疗损害鉴定案例均包含时空观、因果观和量化观的整体-整合医学理念,可作为医学生和临床医生的整合医学知识结构建立和整体医学思维模式培养的典型案例素材,为临床医学教学改革提供新思路。In forensic medical damage identification,common cases of medical malpractice include missed diagnosis,misdiagnosis,improper treatment,and failure to fulfill the duty of care.The primary cause is the lack of integrated medical knowledge structures and holistic medical thinking among clinicians.To date,effective methods for integrating and holistic medical education and continuing education have yet to be established.Based on the evidence advantages of the core forensic work,evidence collection,evidence-based reasoning,and argumentation,this study proposes a holistic-integrative medical approach using the structured thinking model of medical death certification aligned with the International Classification of Diseases(ICD).Each medical damage identification case incorporates the holistic-integrative medical concepts of time-space perspective,causality,and quantification,which can serve as typical case materials for establishing integrative medical knowledge structures and cultivating holistic medical thinking in medical students and clinicians.This approach offers practical and innovative insights for advancing comprehensive reform in clinical medical education.
