摘 要:目的:探究阴道内乳杆菌含量与高危型人乳头瘤病毒(HPV)感染的关系。方法:选取2023年10月至2024年4月在我院门诊就诊的800例患者为研究对象,均同时行阴道分泌物微生态和HPV检测。根据阴道内乳杆菌的含量(菌群密集度)将其分为乳杆菌正常组(Ⅱ~Ⅲ级)400例和乳杆菌少量组(0~Ⅰ级)400例,比较两组HPV的感染率;根据高危型HPV感染情况分为单型HPV感染组155例、多型HPV感染组29例及HPV阴性组496例,进一步分析其阴道菌群微生态情况。结果:800例研究对象中,共184例感染高危型HPV(23.0%)。乳杆菌少量组的高危型HPV感染率高于乳杆菌正常组(34.5%vs 11.5%,P<0.05)。在菌群密集度异常率、乳杆菌群密集度异常率、菌群多样性异常率、pH>4.5、白细胞计数异常率方面,多型HPV感染组高于单型HPV感染组,且两组均高于HPV阴性组(P<0.05)。结论:阴道内乳杆菌含量与HPV感染有关,多型HPV感染更易导致阴道微生态失衡。Objective:To explore the relationship between the content of lactobacillus in the vagina and high-risk human papillomavirus(HPV)infection.Methods:A total of 800 patients who visited the outpatient clinic of our hospital from October 2023 to April 2024 were selected as the research objects.Both vaginal secretion microecology and HPV detection were carried out simultaneously.According to the content of lactobacillus in the vagina(flora density),they were divided into 400 cases in the normal lactobacillus group(gradeⅡ-Ⅲ)and 400 cases in the low lactobacillus group(grade 0-Ⅰ).The infection rates of HPV were compared between the two groups.According to the high-risk HPV infection status,they were further divided into 155 cases in the single-type HPV infection group,29 cases in the multi-type HPV infection group and 496 cases in the HPV-negative group,and the microecological situation of the vaginal flora was further analyzed.Results:Among the 800 research objects,184 cases were infected with high-risk HPV(23.0%).The high-risk HPV infection rate in the low lactobacillus group was higher than that in the normal lactobacillus group(34.5%vs 11.5%,P<0.05).In terms of the abnormal rate of flora density,the abnormal rate of lactobacillus flora density,the abnormal rate of flora diversity,pH>4.5 and the abnormal rate of white blood cell count,the multi-type HPV infection group was higher than the single-type HPV infection group,and both groups were higher than the HPV-negative group(P<0.05).Conclusion:The content of lactobacillus in the vagina is related to HPV infection,and multi-type HPV infection is more likely to lead to vaginal microecological imbalance.
