摘 要:目的构建妇产科住院医师置信职业行为(EPAs)评价体系并进行实证研究,为妇产科住院医师临床能力评价提供更有效的参考。方法采用德尔菲法,对通过文献回顾得到的EPAs条目的重要性和适宜性进行评分,组成EPAs评价框架。由专业基地教学小组按照EPAs评价框架对在培的3个年级68名住院医师进行测评。结果制订由临床常用诊疗技能、手术操作技能、职业素养组成的EPAs评价框架。测评结果显示,二年级妇产科住院医师EPAs得分高于一年级妇产科住院医师(P<0.05),二年级与三年级妇产科住院医师的EPAs得分差异不具有统计学意义(P>0.05)。既往有临床工作经历的一年级妇产科住院医师较无工作经历的一年级妇产科住院医师EPAs得分明显高(P<0.05)。结论构建的EPAs评价体系较好地体现了妇产科住院医师培养特色,以及住院医师胜任力进阶式培养成效,EPAs若要有效运用还需加强师资评价能力和评价标准培训。Objective To construct an evaluation system for the entrustable professional activities(EPAs)of obstetrics and gynecology resident physicians and conduct empirical research,to provide more effective references for the clinical competence evaluation of obstetrics and gynecology resident physicians.Methods The Delphi method was used to score the importance and suitability of EPAs items obtained through literature review,forming an EPAs evaluation framework.The professional base teaching team evaluated 68 resident physicians from three grades under training according to the EPAs evaluation framework.Results Develop an EPAs evaluation framework consisting of commonly with clinical diagnosis and treatment skills,surgical operation skills,and professional ethics.The evaluation results showed that the EPAs scores of second year obstetrics and gynecology resident physicians were higher than those of first year obstetrics and gynecology resident physicians(P<0.05),and the difference in EPAs scores between second and third year obstetrics and gynecology resident physicians was not statistically significant(P>0.05).The EPAs scores of first year obstetrics and gynecology resident physicians with previous clinical work experience were significantly higher than those without work experience(P<0.05).Conclusion The constructed EPAs evaluation system effectively reflects the characteristics of obstetrics and gynecology resident physician training,as well as the effectiveness of advanced competency training for resident physicians.To effectively utilize EPAs,it is necessary to strengthen teacher evaluation capabilities and evaluation standard training.
