摘 要:目的:探讨枯草杆菌二联活菌肠溶胶囊辅助胶体果胶铋治疗溃疡性结肠炎(Ulcerative colitis,UC)患者的效果。方法:回顾性分析2022年2月至2024年3月我院收治的75例UC患者资料。给予两组营养支持、饮食清淡、消炎补液等基础治疗后,以不同治疗方法将患者分为对照组(口服胶体果胶铋100mg·次-1治疗,36例)和实验组(基于对照组口服枯草杆菌二联活菌肠溶胶囊500 mg·次-1辅助治疗,39例),两组均治疗6 m。比较两组的临床疗效、症状消失时间,于治疗前、治疗6 m后采用高倍显微镜观察并计算两组肠道菌群数目,采用酶联免疫吸附法检测两组表皮细胞生长因子(Epidermal growth factor,EGF)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白介素-8(Lnterleukin-8,IL-8)、C-反应蛋白(C-reactive protein,CRP)水平,记录两组治疗期间的不良反应发生情况。结果:实验组的治疗总有效率高于对照组(P<0.05)。两组的不良反应总发生率无显著差异(P>0.05)。实验组的上腹部疼痛、嗳气、腹泻、脓血便消失时间均短于对照组(P<0.05)。两组治疗后的TNF-α、IL-8、CRP水平及肠球菌、大肠埃希菌数目均下降,EGF水平及乳酸杆菌、拟杆菌、双歧杆菌数目均显著上升,且实验组改善幅度大于对照组(P<0.05)。结论:枯草杆菌二联活菌肠溶胶囊辅助胶体果胶铋治疗UC患者,能提高疗效,改善肠道菌群及临床症状,缓解炎症。Objective:To investigate the effect of Bacillus subtilis live bacteria enteric-coated capsules as an adjuvant to colloidal bismuth pectin in the treatment of ulcerative colitis(UC)patients.Methods:Retrospective analysis of the data of 75 UC patients admitted to our hospital from February 2022 to March 2024.After basic treatment such as nutritional support,light diet,and anti-inflammatory fluid infusion,patients were divided into a control group(36 cases)treated with oral colloidal bismuth pectin 100 mg·time-1 and an experimental group(39 cases)treated with oral live subtilis bifidobacterium enteric-coated capsules 500 mg·time-1 as an adjuvant therapy based on the control group.Both groups were treated for 6 months.The clinical efficacy and symptom disappearance time of the two groups were compared.Before and after 6 months of treatment,the number of intestinal florae in the two groups was observed and calculated using high-power microscopy.The levels of epidermal growth factor(EGF),tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),and C-reactive protein(CRP)in the two groups were detected using enzyme-linked immunosorbent assay.The occurrence of adverse reactions during treatment was recorded in both groups.Results:The total effective rate of the experimental group was higher than that of the control group(P<0.05).There was no significant difference in the overall incidence of adverse reactions between the two groups(P>0.05).The disappearance time of upper abdominal pain,belching,diarrhea,and bloody purulent stool in the experimental group was shorter than that in the control group(P<0.05).After treatment,the levels of TNF-α,IL-8,CRP,and the numbers of enterococci and Escherichia coli in both groups decreased,while the levels of EGF and the numbers of lactobacilli,bacteroides,and bifidobacteria significantly increased.The improvement in the experimental group was greater than that in the control group(P<0.05).Conclusion:The treatment of UC patients with Bacillus subtilis live bacteria enteric-coated capsule
