摘 要:目的探讨3 D模型及混合现实全息影像系统在住院医师规范化培训(简称住培)女性压力性尿失禁(stress urinary incontinence,SUI)手术教学中的应用效果。方法利用随机数字表将2021年7月13日到2023年3月30日60名于泌尿外科接受住培的住院医师随机分为观察组和对照组,每组各30例。选取60例女性SUI患者,术前行骨盆CT成像,利用软件生成3 D虚拟模型,打印3 D骨盆模型。观察组利用3 D模型进行骨盆结构的教学,熟悉骨盆解剖,并将其应用于手术前计划、术中解剖定位以及手术入路观察。住院医师佩戴Hololens眼镜,可实现虚拟对象与真实世界交互。对照组按常规方法进行教学,即指导医师讲解幻灯片、解剖图片、手术中操作注意事项等。对比分析两组住院医师的理论水平掌握情况、手术操作熟练程度以及对教学方法的满意度。结果观察组盆底解剖、SUI手术步骤以及注意事项等项目平均成绩、对教学方式的满意度、穿刺成功率均高于对照组,差异有统计学意义(P<0.05),观察组每个项目的答题时长均短于对照组(P<0.01)。结论3 D模型及混合现实全息影像系统可增强住院医师对复杂的盆底解剖结构的认识,有利于提高女性SUI手术的教学效果。Objective To explore the application effect of 3 D models and mixed reality holographic imaging technology in standardized residency training of female stress urinary incontinence.Methods Sixty residents receiving standardized residency training were randomly divided into observation group and control group,with 30 in each group.Sixty female patients with stress urinary incontinence were selected,and preoperative pelvic CT imaging was performed.Their 3 D virtual models were generated using soft scenes,and the 3 D pelvic models were printed.The study group used 3 D models for learning pelvic structures,familiarizing themselves with pelvic anatomy,as well as for preoperative planning,intraoperative anatomical positioning and observation of surgical approaches.They wore Hololens glasses during surgery to achieve interaction between virtual objects and the real world.The control group was taught only with the PPT,anatomical images and operational precautions during surgery using conventional methods for learning.The theoretical level,surgical skills and satisfaction with the teaching methods were analyzed and compared between two groups of resident physicians.Results The average scores of pelvic anatomy,SUI surgical procedures and precautions,as well as satisfaction with teaching methods and the success rate of puncture in the study group were higher than those in the control group(P<0.05).The study group had shorter answer time for each item than the control group(P<0.01).Conclusion Mixed reality holographic imaging technology and 3 D models can enhance the understanding of complex pelvic floor anatomy by resident physicians,which is beneficial for improving the teaching effectiveness of female stress urinary incontinence surgery.
