摘 要:目的探究丹参酮对肛周脓肿大鼠创面愈合及辅助性T细胞1/辅助性T细胞2(Th1/Th2)免疫平衡的作用机制。方法将53只SD大鼠随机分为对照组10只和造模组43只,43只造模组大鼠制备肛周脓肿模型后随机分为模型组及丹参酮低、中、高剂量组,每组10只。丹参酮低、中、高剂量组分别予100、200、300 mg/kg丹参酮溶液10 mL腹腔注射,每日1次;对照组和模型组大鼠予二甲基亚砜10 mL腹腔注射,每日1次。各组大鼠均连续干预14 d。观察各组大鼠创面形态,检测创面愈合率;酶联免疫吸附法检测各组大鼠血清干扰素γ(interferon-γ,IFN-γ)、白细胞介素12(IL-12)、IL-4、IL-10水平;苏木素-伊红(HE)染色观察创面组织病理学变化,并计算成纤维细胞和新生毛细血管数;流式细胞仪检测创面组织中Th细胞亚群分布;蛋白免疫印迹法检测创面组织中Notch1蛋白表达。结果与对照组相比,模型组治疗2、6、10、14 d创面愈合率及治疗后血清IL-4、IL-10水平、新生毛细血管数、成纤维细胞数、Th2细胞比例明显降低(P<0.05),血清IL-12、INF-γ水平、创面组织Th1细胞比例、Th1/Th2比值、Notch1蛋白表达明显升高(P<0.05);与模型组相比,丹参酮低、中、高剂量组治疗2、6、10、14 d创面愈合率及治疗后血清中IL-4、IL-10水平、新生毛细血管数、成纤维细胞数、Th2细胞比例明显增加(P<0.05),血清IL-12、INF-γ水平、创面组织Th1细胞比例和Th1/Th2比值、Notch1蛋白表达明显降低(P<0.05),且呈剂量依赖(P<0.05)。结论丹参酮可减轻肛周脓肿大鼠创面炎症反应,维持Th1/Th2细胞免疫平衡,促进创面愈合,其作用机制可能与抑制Notch1蛋白表达有关。Objective To investigate the mechanism of tanshinone on wound healing and T helper cell 1/T helper cell 2(Th1/Th2)cell immune balance in rats with perianal abscess.Methods A total of 53 Sprague-Dawley(SD)rats were randomly divided into the control group(n=10),and modelling group(n=43).Perianal abscess model was created in the modelling group,and then rats were randomly divided into the model group,and low-dose,medium-dose and high-dose tanshinone groups,with 10 rats in each group.Daily peritoneal injection of 10 mL of dimethyl sulfoxide(DMSO),100 mg/kg,200 mg/kg and 300 mg/kg tanshinone was performed in the above four groups,respectively,for 14 days.The morphology of the wound surface was observed,and the wound healing rate was measured.Serum interferon-γ(IFN-γ),interleukin-12(IL-12),IL-4 and IL-10 were detected by enzyme-linked immunosorbent assay(ELISA).The hematoxylin and eosin(H&E)staining was used to detect pathological changes in the wound tissue,and the number of fibroblasts and newly formed capillaries was calculated.Flow cytometry was used to detect the distribution of Th cell subpopulations in wound tissue.Western blotting was used to detect the protein expression of Notch1 in wound tissue.Results Compared with the control group,the wound healing rate on days 2,6,10,14,serum levels of IL-4 and IL-10,neovascularization,fibroblast count,Th2 cell proportion were significantly lower in the model group,while the serum levels of IL-12 and INF-γ,wound tissue Th1 cell proportion,Th1/Th2 ratio,and protein expression of Notch1 were significantly higher(all P<0.05).Compared with the model group,the low-dose,medium-dose,and high-dose groups showed a significant increase in wound healing rate on days 2,6,10,14,serum levels of IL-4 and IL-10,neovascularization,fibroblast count,Th2 cell proportion,and a significant decrease in serum levels of IL-12 and INF-γ,wound tissue Th1 cell proportion,Th1/Th2 ratio,and protein expression of Notch1 in a dose-dependent manner(all P<0.05).Conclusion Tanshinone can alleviate the
