摘 要:目的探究曲面设计角膜塑形镜和常规角膜塑形镜治疗青少年近视效果。方法回顾性收集2021年1月—2023年5月本院医治的76例青少年近视患者临床资料,将采用常规角膜塑形镜矫正的35例患者列为常规角膜组,以曲面设计角膜塑形镜治疗的41例患者列为曲面角膜组。比较两组手术前后屈光度、裸眼视力和角膜治疗区偏心量、角膜参数[表面非对称性指数(SAI)、角膜散光值(CYL)、表面规则性指数(SRI)]、泪腺状况及不良反应。结果与常规角膜组相比,配戴4周后,曲面角膜组裸眼视力、屈光度较高,角膜治疗区偏心量较低(P<0.05);曲面角膜组SRI较高,SAI、CYL较低(P<0.05);曲面角膜组角膜荧光素染色评分较低,泪液分泌量较多,泪膜破裂时间较长(P<0.05);两组不良反应情况比较,P>0.05。结论曲面设计角膜塑形镜可改善近视患者裸眼视力、屈光度,缩短角膜治疗区偏心量,减少对角膜参数、泪腺状况影响,安全性较好。Objective To explore the effect of curved orthokeratology lens and conventional orthokeratology lens on the treatment of myopia in adolescents.Methods The clinical data of 76 cases of adolescent myopia treated in our hospital from January 2021 to may 2023 were retrospectively collected.35 cases treated with conventional orthokeratology were classified as the conventional corneal group,and 41 cases treated with curved orthokeratology were classified as the curved corneal group.The diopter,naked eye vision,eccentricity of corneal treatment area,corneal parameters[surface asymmetry index(SAI),corneal astigmatism value(CYL),surface regularity index(SRI)],lacrimal gland status and adverse reactions before and after operation were compared between the two groups.Results Compared with the conventional corneal group,after 4 weeks of wearing,the naked vision and refractive power of the curved corneal group were higher,and the eccentricity of the corneal treatment area was lower(P<0.05);the SRI of the curved corneal group was higher,and the SAI and CYL were lower(P<0.05);the corneal fluorescein staining score of the curved corneal group was lower,the tear secretion was higher,and the tear film breakup time was longer(P<0.05);there was no significant difference in the incidence of adverse reactions in the two groups(P>0.05).Conclusion Curved orthokeratology lens can improve the naked eye vision and refractive error,shorten the eccentricity of the corneal treatment area,reduce the impact on corneal parameters and lacrimal gland condition,and have good safety.
