摘 要:目的探讨血管介入手术模拟训练系统在外科住院医师培训中的应用效果。方法选取2022年12月—2023年5月中南大学湘雅二医院外科住院医师44名作为研究对象,随机分为试验组和对照组。对照组采用“传统理论教学+手术带教”模式教学,试验组采用“传统理论教学+血管介入手术模拟训练”教学模式。通过专家评分和调查问卷比较2组住院医师在血管介入手术中造影剂用量、导管(导丝)以及支架释放稳定性、X线透视时长、手术总时长、手术熟练度及术中并发症发生情况、临床技能、培训效果及本次培训的满意度。结果2组的器械选择差异均无统计学意义(P>0.05),试验组的造影剂用量(50.77±10.60)mL、手术总时长(1394.50±114.71)s低于对照组[(66.45±14.33)mL、(1501.80±174.71)s],差异有统计学意义(P<0.05),2组在导管(导丝)稳定性、支架释放稳定性、X线透视时长、手术熟练度方面比较差异均有统计学意义(P<0.05);2组在临床技能、手术熟练度、提高学习兴趣、教学满意度方面比较差异均有统计学意义(P<0.05)。2组的临床思维能力、知识掌握能力、应对术中突发状况、手术决策能力方面比较差异均无统计学意义(P>0.05)。结论应用血管介入手术模拟训练系统辅助教学能够提高外科住院医师的临床操作水平,降低介入手术操作中的辐射时间,减少造影剂的用量,提高手术熟练度。Objective To explore the application effect of vascular interventional surgery simulation training system in the training of surgical residents.Methods A total of 44 surgical residents in the Second Xiangya Hospital of Central South University from December 2022 to May 2023 were selected as the study objects and randomly divided into experimental group and control group.The control group was taught with"traditional theoretical teaching+surgical guidance"teaching mode,while the experimental group was taught with"traditional theoretical teaching+vascular intervention surgery simulation training"teaching mode.The use of contrast agent,catheter(guide wire)and stent release stability,X-ray fluoroscopy time,total operation time,surgical proficiency and intraoperative complications,clinical skills and abilities,training effect and satisfaction of the training were compared between the two groups of residents in vascular interventional surgery by expert scores and questionnaires.Results There was no significant difference in instrument selection between the two groups(P>0.05).The contrast agent dosage of the experimental group(50.77±10.60)mL and the total operation time of the experimental group(1394.50±114.71)s were lower than those of the control group[(66.45±14.33)mL,(1501.80±174.71)s].The differences were statistically significant(P<0.05),and there were statistically significant differences in catheter(guide wire)stability,stent release stability,X-ray fluoroscopy time and surgical proficiency between the two groups(P<0.05).There were significant differences between the two groups in clinical skills,surgical proficiency,learning interest and teaching satisfaction(P<0.05).There were no significant differences in clinical thinking ability,knowledge mastery ability,coping with intraoperative emergencies and surgical decision-making ability between the two groups(P>0.05).Conclusion The application of vascular intervention surgery simulation training system to assist teaching can improve the clinical operation level o
