一起都柏林沙门氏菌食源性疾病暴发事件的调查分析

摘　　要：目的:对食源性疾病暴发事件进行调查和溯源,发现和探索其特征,为研究类似事件提供参考。方法:采用流行病学、食品卫生学和实验室检测及脉冲场凝胶电泳(PFGE)技术等方法。结果:共确认病例85例,罹患率为11.7%(85/727),12份病例粪便标本、2份酒店从业人员肛拭子标本中均检出都柏林沙门氏菌,PFGE方法确定同源;环境标本、清洗用水、蓄水池水、留样食品及部分病例粪便标本检出A组轮状病毒。结论:该起事件是由都柏林沙门氏菌引起的食源性疾病暴发事件,原因食品不明,感染源来自酒店从业人员;存在高危因素—A组轮状病毒,源自二次供水。建议相关监管部门加强对餐饮、二次供水单位的监管,将非接触直接入口食品工作的人员也纳入健康管理,增设二次供水的监测机制,防止此类事件再次发生。OBJECTIVE To investigate and trace the source of mixed-factor foodborne disease outbreak events,discover and explore its characteristics,and provide reference for the study of similar events.METHODS Through epidemiology,food hygiene and laboratory testing and pulsed field gel electrophoresis(PFGE)technology.RESULTS 85 cases were confirmed,with the incidence rate of 11.7%(85/727)cases,salmonella Dublin was detected in 12 patient stool specimens and 2 anal swabs from hotel staff,and PFGE method determine the homology;rotavirus group in environmental samples,cleaning water,reservoir water,sample food and stool samples of some cases.CONCLUSION The incident was an outbreak of a foodborne illness caused by Salmonella in Dublin,food for transmitting the disease is unknown,the source of the infection came from the hotel staff.Co-existing high-risk factors:group A rotavirus,it originates from the secondary water supply.It is suggested that the relevant regulatory authorities should strengthen the supervision of catering and secondary water supply units,including people working in non-contact ready-to-eat food products into health management,add a monitoring mechanism for secondary water supply,to prevent such incidents from happening again.
