父母行为训练疗法联合托莫西汀治疗学龄期儿童注意缺陷多动障碍的效果观察

摘  要:目的探讨父母行为训练疗法联合口服托莫西汀对学龄期注意缺陷多动障碍(ADHD)患儿智力水平的影响。方法采取随机数字表法将2021年1月~2023年12月选取的168例ADHD患儿分为对照组(n=84)与观察组(n=84)。对照组仅给予口服托莫西汀治疗,观察组在此基础上联合父母行为训练疗法。治疗前后分别采用视听整合持续测试系统(IVA-CPT)评估患儿反应力及注意力,韦氏儿童智力量表(WISC)评估患儿智力水平,Conners父母症状问卷(PSQ)评估认知行为,儿童感觉统合评定量表(CSIRS)评估感觉统合能力,儿童社交焦虑量表(SASC)评估社交功能。结果治疗后,观察组IVA-CPT评分中的综合反应控制商数(91.57±2.11)分、综合注意力商数(87.98±2.38)分,均高于对照组(85.78±2.75)分、(81.21±3.47)分(P<0.05)。治疗后,观察组各维度WISC评分、CSIRS评分高于对照组(P<0.05),PSQ评分低于对照组(P<0.05)。治疗后,观察组SASC评分中的害怕与否定(6.61±2.41)分、社交回避及苦恼(11.21±3.41)分,均低于对照组的(9.59±3.25)分、(14.69±3.65)分(P<0.05)。结论对于学龄期ADHD儿童,父母行为训练疗法联合口服托莫西汀治疗,可有效提高其智力水平,改善认知行为、感觉统合能力及社交功能。Objective To explore the effects of parental behavioral training therapy combined with oral tomoxetine on intelligence level in school-age children with attention deficit hyperactivity disorder(ADHD).Methods According to random number table method,168 children with ADHD were divided into the control group(n=84,oral tomoxetine)and the observation group(n=84,parental behavioral training therapy combined with oral tomoxetine)between January 2021 and December 2023.Before and after treatment,reactivity and attention of children were evaluated by integrated visual and auditory continuous performance test(IVA-CPT),intelligence level was evaluated by wechsler intelligence scale for children(WISC),cognitive behaviors were evaluated by conners parent symptom questionnaire(PSQ),sensory integration ability was evaluated by children sensory integration rating scale(CSIRS),and social function was evaluated by social anxiety scale for children(SASC).Results After treatment,scores of IVA-CPT(comprehensive reaction control quotient,comprehensive attention quotient)in the observation group were(91.57±2.11)points and(87.98±2.38)points,higher than those in control group[(85.78±2.75)points,(81.21±3.47)points,P<0.05].After treatment,scores of WISC and CSIRS in observation group were higher than those in control group(P<0.05),and PSQ score was lower than that in control group(P<0.05).After treatment,scores of SASC(fear and negation,social avoidance and distress)in the observation group were(6.61±2.41)points and(11.21±3.41)points,lower than those in the control group[(9.59±3.25)points,(14.69±3.65)points,P<0.05].Conclusion Parental behavioral training therapy combined with oral tomoxetine can effectively improve intelligence level,cognitive behaviors,sensory integration ability and social function in school-age children with ADHD.
