基于结构方程模型的青少年抑郁症患者自杀意念影响路径分析

摘　　要：目的探讨青少年抑郁症患者自杀意念现状,并分析其影响路径。方法采用方便抽样法,选取2022年1月-2024年7月我院收治的青少年抑郁症患者280例为研究对象,采用一般资料调查表、中文版Beck自杀意念量表(BSI-CV)、中文版心理弹性量表(CD-RISC)、中文版童年期创伤问卷(CTQ)、汉密尔顿抑郁量表(HAMD)、青少年生活事件量表(ASLEC)、家庭功能评定量表(FAD)进行调查。影响因素采用多元线性回归分析,采用AMOS 24.0软件构建结构方程模型,分析青少年抑郁症患者自杀意念的影响路径。结果280例青少年抑郁症患者中165例存在自杀意念,自杀意念发生率为58.93%;多元线性回归分析结果显示,校园欺凌、病耻感、FAD评分、CD-RISC评分、CTQ评分、HAMD评分、ASLEC评分是青少年抑郁症患者自杀意念的影响因素(P<0.05);结构方程模型显示,校园欺凌、CD-RISC评分、HAMD评分对青少年抑郁症患者自杀意念的影响路径均为直接效应,其中校园欺凌和HAMD评分可直接正向影响自杀意念,CD-RISC评分可直接负向影响自杀意念(P<0.05);病耻感对青少年抑郁症患者自杀意念的影响路径为直接效应和间接效应,可直接正向影响自杀意念,亦可通过CD-RISC评分对患者自杀意念产生间接影响(P<0.05);FAD、CTQ及ASLEC评分对青少年抑郁症患者自杀意念的影响路径为直接效应和间接效应,均可直接正向影响自杀意念,亦可通过CD-RISC、HAMD评分对自杀意念产生间接影响(P<0.05)。结论青少年抑郁症患者产生自杀意念受多种因素影响,医护人员应根据影响因素及时进行心理干预,引导患者积极应对不良事件及负性情绪,提升患者的心理弹性水平,降低自杀意念风险。Objective To explore the current status of suicidal ideation in adolescent depression patients and analyze its impact pathways.Methods Convenience sampling was used to select 280 adolescent depression patients admitted to our hospital from January 2022 to July 2024 as the research subjects.A general survey questionnaire,the Chinese version of the Beck Suicidal Ideation Assessment Scale(BSI-CV),the Psychological Resilience Scale(CD-RISC),the Childhood Trauma Questionnaire(CTQ),the Hamilton Depression Scale(HAMD),the Adolescent Life Events Scale(ASLEC),and the Family Function Assessment Device(FAD)were used for investigation.The influencing factors were analyzed using multiple linear regression,and a structural equation model was constructed using AMOS 24.0 software to analyze the impact pathway of suicidal ideation in adolescent depression patients.Results Out of 280 adolescent depression patients,165 had suicidal ideation,with an incidence rate of 58.93%;The results of multiple linear regression analysis showed that campus bullying,shame,FAD score,CD-RISC score,CTQ score,HAMD score,and ASLEC score were the influencing factors of suicidal ideation in adolescent depression patients(P<0.05);The structural equation model showed that campus bullying,CD-RISC score and HAMD score all had direct effects on suicidal ideation in adolescents with depression,among which campus bullying and HAMD score had direct positive effects on suicidal ideation,while CD-RISC score had direct negative effects on suicidal ideation(P<0.05).The influence paths of stigma on suicidal ideation of adolescents with depression were direct effect and indirect effect,which could directly positively affect suicidal ideation and indirectly affect suicidal ideation by influencing CD-RISC score(P<0.05).The influence paths of FAD,CTQ and ASLEC on suicidal ideation in adolescents with depression are direct effect and indirect effect,which can directly affect suicidal ideation,and indirectly affect suicidal ideation by influencing CD-RISC score or HAMD sco
