摘  要:目的探讨谷胱甘肽过氧化酶4(GPX4)基因单核苷酸多态性(SNP)在非贲门胃癌发病风险中的作用。方法选取1031例对象,包括506例正常体检者和525例非贲门胃癌患者;对GPX4基因rs4807542、rs713041、rs2074451、rs3746162四个位点使用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)的方法进行基因分型;用非条件性Logistic回归计算各位点在4种遗传模型下与非贲门胃癌发病风险的关联性。结果rs713041的CT+TT基因型相对于CC基因型而言降低了非贲门胃癌的发病风险(OR=0.699,95%CI:0.537~0.910);rs2074451的GT+TT基因型相对于GG基因型而言降低了非贲门胃癌的发病风险(OR=0.681,95%CI:0.520~0.893);由GPX4四个SNP构建的G-C-G-C单体型与非贲门胃癌发病风险增加相关(OR=1.262,95%CI:1.035~1.539),G-T-T-C单体型与非贲门胃癌发病风险降低相关(OR=0.784,95%CI:0.656~0.937);GPX4 rs4807542、rs713041、rs2074451、rs3746162的四阶交互作用在非贲门胃癌的发生风险中起到了协同效应(P<0.05)。结论GPX4 rs713041、rs2074451与非贲门胃癌易感性相关;由GPX4 rs4807542、rs713041、rs2074451、rs3746162构成的G-C-G-C单体型是非贲门胃癌发病的危险因素,而G-T-T-C单体型是保护因素;且GPX4 rs4807542、rs713041、rs2074451、rs3746162的交互作用与非贲门胃癌的发生密切相关。Objective To investigate the role of single nucleotide polymorphisms(SNP)of glutathione peroxidase 4(GPX4)gene in the risk of non-cardia gastric cancer.Methods A total of 1031 samples were selected,including 506 normal examiners and 525 patients with non-cardia gastric cancer.GPX4 rs4807542,rs713041,rs2074451 and rs3746162 were genotyped by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).Unconditional Logistic regression was used to analyze the relationship between each SNP with the risk of non-cardia gastric cancer under the four genetic models.Results The CT+TT genotype of rs713041 reduced the risk of non-cardia gastric cancer compared with the CC genotype(OR=0.699,95%CI:0.537-0.910).The GT+TT genotype of rs2074451 reduced the risk of non-cardia gastric cancer compared with the GG genotype(OR=0.681,95%CI:0.520-0.893).The G-C-G-C haplotype,constructed by the four SNPs of GPX4,was related to an increased risk of non-cardia gastric cancer(OR=1.262,95%CI:1.035-1.539),and G-T-T-C haplotype was related to a decreased risk of non-cardia gastric cancer(OR=0.784,95%CI:0.656-0.937).The fourth-order interaction of GPX4 rs4807542,rs713041,rs2074451 and rs3746162 played a synergistic effect in the risk of non-cardia gastric cancer(P<0.05).Conclusion GPX4 rs713041 and rs2074451 are related to non-cardia gastric cancer susceptibility.The G-C-G-C haplotype composed of GPX4 rs4807542,rs713041,rs2074451 and rs3746162 is a risk factor for non-cardia gastric cancer,while the G-T-T-C haplotype is a protective factor.The interaction between GPX4 rs4807542,rs713041,rs2074451 and rs3746162 is closely connected with the occurrence of non-cardia gastric cancer.
