管理关怀感知在急诊科护士心理复原力与创伤后应激障碍中的中介作用

摘　　要：目的探讨管理关怀感知在急诊科护士心理复原力与创伤后应激障碍(PTSD)之间的中介作用,为制订针对性措施来改善护士心理健康提供参考依据。方法于2024年1-3月选取该院急诊科护士为研究对象进行方便抽样,采用一般资料调查表、创伤性应激障碍自评量表、心理复原力量表、中文版管理关怀评估量表进行调查。结果165名急诊科护士PTSD得分(49.25±5.84)分,阳性检出率为46.06%(76/165);Pearson相关分析显示,心理复原力、管理关怀感知均与PTSD呈负相关(r=-0.497、-0.595,P<0.001),管理关怀感知与心理复原力呈正相关(r=0.495,P<0.001);管理关怀感知在心理复原力与PTSD之间起部分中介作用,中介效应量为41.83%;心理复原力通过管理关怀感知对PTSD变量的解释由31.00%上升至43.60%,对PTSD变量的负向预测强度由-0.447下降至-0.298;结构方程模型分析显示,管理关怀感知是心理复原力和PTSD的中介变量,模型拟合指标良好。结论急诊科护士的PTSD与管理关怀感知、心理复原力相关,管理关怀感知通过对心理复原力进行正向调节,能够降低PTSD水平。Objective To explore the mediating role of perceived management care on psychological resilience and post-traumatic stress disorder(PTSD)of nurses in emergency department,and to provide reference for formulating targeted measures to improve the mental health of nurses.Methods From January to March 2024,a convenient sampling was conducted to select nurses in the emergency department of our hospital as the research objects.The general information questionnaire,traumatic stress disorder self-rating scale,psychological resilience scale and Chinese version of management care assessment scale were used for investigation.Results The PTSD scores of 165 emergency department nurses was(49.25±5.84)points,and the positive detection rate was 46.06%(76/165).Pearson correlation analysis showed that psychological resilience and perceived management care were negatively correlated with PTSD(r=-0.497,-0.595,P<0.001),and perceived management care was positively correlated with psychological resilience(r=0.495,P<0.001).Perceived management care played a partial mediating role between psychological resilience and PTSD,and the mediating effect size was 41.83%.The interpretation of psychological resilience on PTSD variables through perceived management care increased from 31.00%to 43.60%,and the negative predictive strength to PTSD variables decreased from-0.447 to-0.298.Conclusion The PTSD of emergency department nurses is related to the perceived management care and psychological resilience.The perceived management care can reduce the level of PTSD by positively regulating psychological resilience.
