摘　　要：目的　探讨p16基因在老年胆囊癌的表达。方法　采用免疫组织化学方法对 2 7例老年胆囊癌标本p16基因表达进行检测。结果　老年胆囊癌p16蛋白阳性表达明显低于胆囊良性病变( R胆囊癌 =0 .5 0 0 0 , R胆囊腺癌 =0 .72 5 6 , R胆囊结石 =0 .836 8, R胆囊息肉 =0 .76 17) ;p16蛋白阳性表达与病理类型差异无显著性意义 ( R硬癌 =0 .5 0 0 0 , R胆囊息肉 =0 .5 132 , R粘液癌 =0 .4 6 0 6 ) ;p16蛋白阳性表达与Nevin临床分期差异有显著性意义 ( RS1 - 2 =0 .5 0 0 0 , RS3=0 .2 6 14 , RS4 - 5=0 .12 5 0 ) ,S1 - 2 p16蛋白阳性表达明显高于S3、S4 - 5。Objective To study the expression of p16 in aged gallbladder carcinoma. Methods The expression of p16 in 27 aged gallbladder carcinomas were observed by immunohistochemical method. Results The positive rate of p16 expression was 48.1%, 81.8%, 86.7% and 100% in aged gallbladder carcinoma, gallbladder adenoma, gallbladder polyp and gallbladder stone, respectively ( carcinoma =0.5000, adenoma =0.7256, stone =0.8368, polyp =0.7617). The positive rate of p16 in aged gallbladder carcinoma was significantly lower than that in benign gallbladder diseases. The positive rate of p16 expression was 47.4%, 50.0% and 50.0% in aged gallbladder cirrhotic carcinoma, papillary form adenocarcinoma and mucoid carcinoma, there was no significant difference ( scirrhous carcinoma =0.5000, papillary form adenocarcinoma =0.5132, mucoid carcinoma =0.4606). The positive rate of p16 expression was significantly higher in S 1-2 , than in S 3 and S 4-5 (81.8% vs 37.5% and 12.5%) ( RS1-2 =0.5000, S3 =0.2614, S4-5 =0.1250). Conclusions The mutation and deletion of p16 gene was important in aged gallbladder carcinoma.
