摘 要:目的 评价电化学疗法 (EChT)对大鼠种植型肝癌的治疗效果。材料与方法 采用直接注射法制作大鼠肝癌模型。应用影像学、病理学及系列免疫指标 (IL 2、sIL 2R、IL 6、TNF α)检测 ,生存期比较等方法观察荷瘤大鼠EChT治疗前后肿瘤大小、坏死、细胞凋亡、外周血系列免疫指标表达水平、生存期长短的动态变化 ,并与同期对照组进行比较。结果 EChT治疗前 ,大鼠肝肿瘤平均体积为 10 0± 6mm3 ;EChT治疗后 1周 ,大鼠肝肿瘤平均体积为12 5± 10mm3 ,而荷瘤对照组肝肿瘤平均体积为 190± 11mm3 ;治疗组肿瘤生长率为 2 5 % ,而荷瘤对照组肝肿瘤生长率为 90 % (P <0 .0 5 )。治疗组病理光镜下肿瘤明显坏死 ,并可明显诱发细胞凋亡。荷瘤大鼠EChT治疗前IL 2水平呈低表达 ,sIL 2R、IL 6、TNF α则高表达 ,与正常大鼠比较差异显著 (P <0 .0 5 ) ;EChT治疗后 ,与同期荷瘤大鼠对照组比较 ,IL 2表达水平升高 ,sIL 2R、IL 6、TNF α表达水平下降 (P <0 .0 5 )。EChT治疗组大鼠生存期明显延长 (P <0 .0 5 )。结论 肿瘤负荷可导致大鼠细胞因子表达紊乱。EChT治疗可明显抑制肿瘤生长 ,促进肿瘤坏死 ,诱发肿瘤细胞凋亡 ,改善和纠正机体免疫紊乱 ,增强大鼠免疫力 ,延长大鼠生存期。Objective To make a comprehensive evaluation of electrochemotherapy (EChT) in treating implanted liver tumor in experimental rats.Materials and Methods The liver tumor model of rats was prepared by direct injection of cancerous ascites. Imaging and pathological observation on the size, necrosis and apoptosis of the tumor before and after EChT, the estimation of the survival period, and the measurement of the expression levels of a series of immune index, including IL 2, sIL 2R, IL 6, TNF α, were conducted. The results were compared with that obtained from the control group during the same research period.Results Before EChT, the mean liver tumor volume of the rats was 100±6mm 3. One week after EChT, the mean tumor volume and the growth rate in EChT group and in control group were 125±10mm 3, 25% and 190±11mm 3, 90%, respectively (P< 0.05). Marked necrosis and apoptosis were seen in EChT group, which was more severe than that seen in control group. Before EChT, IL 2 level in rats with tumor was lower than that in normal rats, while sIL 2R, IL 6, TNF α in rats with tumor were higher than those in normal rats. After EChT, IL 2 level was significantly increased (P<0.05), while sIL 2R, IL 6, TNF α were significantly decreased (P<0.05) in rats of tumor group. The survival period in EChT group was longer than that in control group (P<0.05). Conclusion Liver tumor may induce abnormal expression of serum cytokines. EChT can inhibit tumor's growth, promote tumor's necrosis, induce apoptosis, improve and correct immune status, enhance immune level and prolong the survival time in rats.
