摘 要:目的 研究老年重症肌无力(MG)的临床特点。方法 回顾性分析1990-01-2002-05北京医院123例老年MG住院患者的临床特点,并与507例青壮年MG患者进行对比。结果 老年MG患者占同期MG患者的18.14%,男性患者较多(占68.29%),眼肌首发症状多见(占80.49%),分型以Ⅰ型和Ⅱ a型多见(占56.10%),胸腺异常主要为胸腺瘤(占老年MG患者手术病理证实伴发胸腺异常的100%),低频重复电刺激(RNS)阳性率为74.19%,老年MG危象发生率较低(为2.44%)。结论 老年MG患者具有男性、眼肌首发症状、Ⅰ型与ⅡA型多见,以及胸腺异常主要为胸腺瘤,而RNS阳性率与MG危象发生率低等临床特点。Objective To study clinical characteristics of elderly patients with myasthenia gravis (MG). Methods The clinical characteristics of 123 elderly patients with MG admitted to department of neurology, Beijing Hospital from Jan 1990 to May 2002 were retrospectively studied and compared with 507 young adult patients with MG admitted to the same department during the same period. Results The percentage of elderly patient with MG was 18. 14% . Male (68. 29%) was higher than female (31. 71%). The initial symptom of extra ocular muscles involvement was higher (80. 49%). Type Ⅰ and type Ⅱ a were higher (56. 10%). Thymoma was more (100%) in thymic abnormalities. The positive rate of repetitive electric nerve stimulation(RNS) was 74. 19%. Occurrence rate of crises in MG was lower (2. 44%). Conclusions The prevalence of MG in elderly is increasing in males. The initial symptom in majority of patients was ocular muscle involvement. The frequency of type Ⅰ, type Ⅱ a was higher, and abnormality of thymoma was mainly, the positive rate of RNS was lower in young adult MG patients, the occurrence rate of myasthenia crisis was lower. Myasthenia gravis in elderly patients had their own specific clinical features.
