首发急性脑卒中病人出院时情感及日常生活活动能力的临床研究

摘　　要：目的 :探讨综合性医院早期综合康复治疗对急性脑卒中病人情感及日常生活活动能力 (ADL)的影响。方法 :选择住院的首发颈内动脉系统急性脑卒中病人 80例 ,随机分为康复组与对照组 (每组 40例 ) ,康复组在临床药物治疗的同时进行综合康复治疗 ,对照组仅给予药物治疗。两组病人治疗前及出院时心理状况评估采用SCL -90量表 ,日常生活活动能力的评定采用Barthel指数。结果 :经治疗后出院时康复组SCL -90量表各症状因子分均得到了明显的改善 (P <0 .0 5～ 0 .0 1) ,对照组躯体化、强迫、抑郁、焦虑及精神病性等也有明显的改善 (P <0 .0 5 ) ;康复组躯体化、强迫、抑郁、焦虑、恐怖及精神病性等症状改善比对照组更为明显(P <0 .0 5 )。两组病人Barthel指数均有所改善 (P <0 .0 5～ 0 .0 1) ,康复组比对照组改善尤为突出。结论 :急性脑卒中病人早期进行综合康复治疗 ,可进一步改善病人的生活质量。Objective:To study the effect of early colligate rehabilitation on sensibility and activity of daily living(ADL) of the first stroke patients in synthesis hospital. Methods:80 patients of arteriae carotis interna pathological changes were randomly divided into two groups, rehabilitation group(40 cases) and control group(40 cases). Patients in the rehabilitation group were given clinical treatment and colligate rehabilitation training, while those in control group were given clinical treatment, and the evaluation was done in pre-treatment and leave hospital respectively. The psychologic status were tested by the symptom checklis(SCL-90), ADL assessed in Barthel Index. Results:SCL-90 scores in the rehabilitation group amend after treatment(P<0.05～0.01), and control group were amend in somatization,compulsion,depression,anxiety, lunacy etc(P<0.05); the rehabilitation group after treatment were obviously amend in somatization, compulsion, depression, anxiety, panic, lunacy etc to those in the control group(P<0.05). The ADL scores(Barthel Index) in the two group amend after treatment(P<0.05～0.01), and the rehabilitation group after treatment were obviously amend to those in the control group(P<0.05). Conclusion:Early colligate rehabilitation training in urgent stroke may obviously improve the quality of living. Synthesis hospital should high recognition.
