MRA在脑血管病中应用价值的初步探讨

摘　　要：目的 初步探讨磁共振血管造影(MRA)对脑血管病(CVD)诊断及疗效评价等方面的价值。方法 对33例CVD患者同时行磁共振成像(MRI)及MRA检查,从其影像学特点及临床表现进行分析。结果 脑梗塞15例,14例MRA确诊,1例漏诊;颅内动脉瘤6例,MRA检出4例,2例漏诊;4例动静脉畸形(AVM),3例MRA确诊,1例漏诊;2例海绵状血管瘤MRA均漏诊;1例静脉血管瘤MRA确诊;2例烟雾病MRA均确诊;3例脑出血MRA确诊。MRA评价4例脑梗塞疗效,效佳1例,欠佳3例¨例AVM血管内栓塞疗效佳;另1例AVM行X刀手术效差。结论MRA可提供颅内动脉瘤、脑血管畸形、脑大血管的狭窄、闭塞及侧支血流的证据。MRI和MRA同时检查能提高单项MRA对CVD诊断的准确率,为临床治疗提供更多的信息。Purpose Preliminary MR angiography (MRA) investigation of value of diagnosis and assessment of treatment effect of cerebro-vasvular disease (CVD). Methods Both MRA and MR imaging (MRI) were performed in 33 patients with CVD. Imaging characteristic and clinical appearance were analysed. Results 15patients suffered from brain infarction, with 14 patients MRA accurate diagnosis, one misdiagnosis. Four of 6 patients with intracranial aneurysm were diagnosed by MRA and 2 patients misdiagnosed. Three of 4 patients with arteriovenous malformation (AVM) were accurately diagnosed by MRA and one misdiagnosed. 2 patients with caremous angioma were misdiagnosed by MRA. 2 patients with moyamoya disease, one patients with venous angioma and 3 patients with cerebral hemorrhage were accurately diagnosed by MRA. Treatment effect was judged by comparative MRA before and affer treatment. One of 4 patients with cerebral infarction treated by drugs had good effect, the others no good. One AVM treated by endovascular embolization yielded very good effect. The another AVM showed no good effect after being treated by X - Knife. Conclusion MRA may give blood flow certification. MRA combined with MRI can effectively arise the diagmostic accuracy of CVD and give more information for clinical treatment of CVD.
